Job summary
Job Summary:
The role would involve the care co-ordination of a number of Service Users. You will also be involved in assessments and delivering a range of interventions to both individuals and their families and to groups.
You will have the opportunity to work alongside many other clinicians including family workers, CBT therapists, consultant psychiatrists and psychologists.
Driving is a must and your own transport is essential for this role as supporting clients to appointments is part of the work. The team does work flexibly in order to reach clients and carers so some working outside of office hours at times is to be expected. In return you will get to work as part of a small MDT team with a real team approach. There may be opportunities in the future to train in psychological interventions to support your practice. There will be opportunities to network and join in shared learning with other recovery teams, both in AWP and in the South West.
Main duties of the job
Job Purpose:
To undertake the full range of activities required to deliver ongoing comprehensive mental health assessment for service users with severe and enduring mental health needs living in the community and in a range of settings. This will include:
- The use of standardised assessment tools, i.e. the Cluster Allocation Support Tool (CAST), KGV.
- Recovery Star.
- History, strengths and aspirations.
- Mental state.
- Impact of culture and diversity.
- Functional needs.
- The needs of family and carer.
- Evaluation of risk.
- Physical health.
- Complicating factors.
- The interventions and treatments required to enable positive change.
- Social care needs.
- Safeguarding and public protection.
- Capacity under the Mental Capacity Act.
