Job summary

The Wiltshire Dementia Service Complex Care Pathway, provides psychosocial interventions to people living with dementia in the community. The team supports individuals and family/carers within their own homes. Liaising with a variety of professionals, teams and services to facilitate care delivery.

The complex care team sits within the Wiltshire Dementia Service. The service as a whole includes the memory service, Complex Care Pathway and Care Home Liaison Service, allowing seamless transition between the three teams depending on need. The complex care team is a multi-disciplinary team.

We offer monthly supervision annual appraisal, professional and personal development and are committed to the health and well-being of our staff.

Main duties of the job

The Complex Care Pathway is a specialist dementia team providing safe and supportive mental health care for adults with a diagnosis of dementia who are experiencing some level of distress or difficulty due to their illness.

The Team provides a range of community-based health interventions for service users, working in an inclusive way to resolve the immediate difficulties and improve their situation, you will act as a care co-ordinator.

Where appropriate, the post holder will work alongside other services to plan, deliver and review effective interventions to manage risk and prevent hospital admission.

Please note the Wiltshire Dementia Service covers a large geographical area so applicants must have a valid driving licence and access to own transport. It is not possible to carry out the requirements of this role by using public transport.

We welcome applicants from band 5 practitioners with relevant post graduate experience, we will also consider band5/6 development post.
