Job summary
We have a brand new, exciting opportunity for a clinically experienced pharmacist to join the pharmacy teams at Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) and the Royal United Hospitals Bath NHS Foundation Trust (RUH). The successful postholder will work in a post split between leading the mental health pharmacy services for AWP Bath & North East Somerset (BaNES), and developing and leading the provision of a high quality medicines optimisation service as part of RUH Psychiatric Liaison.
The successful candidate will work closely with both AWP lead pharmacists and senior clinicians, as well as the RUH Chief Pharmacist and liaison psychiatry clinicians. We are looking for a highly motivated, innovative and enthusiastic individual that will help shape this new and exciting role, to ensure patients living with mental health conditions in the BaNES locality receive seamless, safe, high quality care, regardless of the setting they are currently in.
This is a wonderful opportunity for a clinical pharmacist to develop the 4 pillars of advanced pharmacy practice and progress their career in a highly specialist area, with the company and support of two excellent pharmacy teams. The successful candidate will be supported to identify and access additional training and development if needed, in order to fulfil the requirements of the role. This includes, but is not limited to:
- Non-medical prescribing
- Leadership courses
- Specialist mental health training
Main duties of the job
- Lead on the provision of an advanced level of clinical pharmacy services to the inpatient and community mental health teams in BaNES.
- Develop and monitor local clinical pharmacy services to inpatient and community mental health teams in BaNES, continuously seeking ways to improve patient outcomes and experiences.
- Lead the local clinical pharmacy team to ensure inpatient units and community teams receive regular high quality clinical pharmacy support appropriate to the needs of the service and service users.
- Develop, plan, manage and lead the provision of a high quality medicines optimisation service to the Psychiatric Liaison (PL) service within RUH FT.
- Provide an expert, specialist medication referral point for healthcare professionals working in the PL teams and be a core multidisciplinary team member, as necessary, to optimise the care of patients and their families.
- Provide advice on ongoing monitoring and management of mental health problems and medication affecting physical healthcare while the patient remains in the acute hospital.
- Agree and deliver outcome targets/KPIs as defined by the PL service and the RUH medication safety agenda.
