Job summary
We are looking for an experienced Band 6 Social worker, OT or nurse to join our PCLS team in South Gloucestershire. We are based at the Kingswood Civic centre, along with secondary mental health team's.

There will be opportunities to have a key role in service development and applicants who are able to think creatively about how mental health services can be delivered are welcomed.
This service currently affords rapid access to credible specialist mental health expertise for GPs, Primary Care staff and other referring agents who are requiring support and advice on meeting the mental health needs of the people within their caseloads. PCLS is a multi-disciplinary team who work together and support each other with a shared caseload of services users, ensuring that each person receives a holistic approach to assessment, brief intervention and signposting.
The successful applicant will hold an appropriate professional registration.
Main duties of the job
Our service aims to ensure that patients in Primary Care have their mental health needs more clearly assessed, defined, cared for and treated by Primary Care Health Care Professionals and that the pathways, to best meet service users' needs are optimised.
The mental health liaison practitioner will prioritize assessments based on clinical need and risk, making evidence-based expert recommendations for care and treatment within and across multiple care pathways, ranging from those within primary care and where necessary into secondary specialist mental health services.
