Job summary
The team will comprise of 3 practitioners who will carry a caseload of 10 - 15 services users each and will be delivering care within the CPA and in partnership with other teams. The practitioners will be integrated within both Recovery teams, providing mutual support where needed and will work alongside existing care co-ordinators and RC.
The service user group will comprise of those with ongoing or recurrent psychosis and affective disorders, whose lives are severely impacted by their symptoms. They may present with instability in many aspects of their lives, a high level of risk and problematic alcohol or drug use; they may also have accommodation issues and be vulnerable in their community, and may be involved in the Criminal Justice System.
We are looking for an experienced and creative registered band 7 mental health nurse to take on a leadership role, including supervision and co-ordinating the service. You will be flexible and energetic, working across the South Glos area and supporting your colleagues within the Recovery service. You will have significant experience of working in mental health in the community.
Main duties of the job
South Gloucestershire are proud to be launching a new service which will sit within its Recovery teams, based at Kingswood Civic Centre and The Elms, in Thornbury. The post will primarily be based in Kingswood.
The Recovery Outreach Support and Engagement team (ROSE) will work with those service users who have historically experienced barriers to accessing services. The focus of the team's work will be on improving individual service user's quality of life by drawing on knowledge of a wide range of interventions.
As well as a focus on improving lives, the service also aims to reduce hospital admissions for this service user group, to improve the frequency and engagement with physical health monitoring, to evidence the offers we can make in terms of support from our colleagues in voluntary services and to reduce service user contact with the Intensive Support Team.
