Be the first to launch this new team, Lead transformation of mental health alliances in Weston Worle and villages and create personalised mental health support for people in primary care.
This is an amazing opportunity. Weston Worle and Villages are launching the first Community integrated mental health Hub IMMH. The IMMH is an integrated team of partners who include primary care, secondary care. Local authority, IAPT, VCSE alliance among others who will work with a shared caseload using the 'team around me approach' and 'trusted assessment' approach to ensure that citizens are at the centre of their person-centred plan, with the person included in all decision making. Our strong alliance will ensure that the people we work with will coordinate their own needs based on what they feel is important to them with our partnered support.
We are the first primary care integrated mental heath team in BNSSG and we are looking for a team coordinator.
Main duties of the job
To lead a multi agency partnership team providing personalised holistic support pathways for individuals in primary care.
To lead and develop integrated services to fill the gap between primary and secondary care.
To support triage of referrals and determine support pathways for individuals seeking mental health support and support your partnership team to develop the " team around me" approach.
To support and coordinate support conversations, trusted assessment, MDTS, Risk assessments, shared caseloads, reviews and audits.
To maintain excellent clinical standards, and effective caseload supervision, underpinned by robust and effective individual and team supervision processes.
To develop systems and processes for the effective management and monitoring of the team caseload, on a day to day basis, ensuring that time is prioritised effectively, and service users and carer receive the appropriate level of service.
To provide a leadership role within the team
