Job summary

We are looking for an enthusiastic and committed Practitioner to join our well-established Care Home Liaison Service in South Gloucestershire. We specialize in delivering high quality interventions to people residing in care homes in the South glos area.

We are a team of Nurses, OT, & Support Workers who work with service users living in care homes who have a diagnosis of dementia and/or functional mental health illness. We work with residents individually, and with families & care staff, and we have a broad range of clinical and therapeutic interests. The successful applicant will provide specialist assessment, formulation, intervention and evaluation, alongside staff training, supervision, consultation, advice and support to the teams they are working within.

To work as an independent practitioner delivering the following core liaison service domains for Older Adults within Residential/Nursing/Care establishments across south Glos:

  • Undertake urgent specialist Mental Health Assessments
  • Provide recommendations for care & treatment within multiple care pathways
  • Determine the nature & severity of mental health needs and provide sign posting and advice about onward care pathways
  • Provide ongoing support, & advice to care home staff
  • Provide ongoing education & training for care home staff

Main duties of the job

To actively support residents in care establishments across South Gloucestershire to maintain their placement and/or avoid an admission to an acute ward. The team promotes the health and well-being of older people using a person centred approach. Individual assessment of the person's needs ensuring a focussed response to issues raised and informs a plan that will provide support and education for staff and informal carers.

To liaise with other members of the multidisciplinary team and other agencies involved in the care of individual service users. Signposting service users and carers to other services as appropriate.

To contribute to integrated governance processes, including the continuing development, improvement and monitoring of service provision and performance, and monitoring standards on care, through data collection and audit processes.

Our base is in Kingswood, S Gloucestershire, on the periphery of the City of Bristol with easy access to the M4 and m5 motorways.

Because the role involves working within the community, a full driving license and access to a car is essential.

To make effective use of the standard liaison approach and structure for assessments according to the SBAR (Situation-Background-Assessment-Recommendation) format
