Confidential and comprehensive mental health nursing assessment that includes a Risk Assessment and a Care Programme within ICPA along with Risk Management Plan
To contribute to medical, nursing and multi-disciplinary care planning providing comprehensive information for determining the distribution of the Community Care Budget.
Following assessment, to refer on to other health services for further assessment in order to clarify further service requirements for individuals.
To facilitate prompt referral to other health, social and voluntary services as appropriate.
To use evidence based practices and psychosocial interventions to assess, plan, implement and evaluate nursing care.
Maintains up to date knowledge of relevant medicines, related medication management issues and acts in accordance with the Guidelines for the administration of medicines (UKCC 2000) and the National Institute for Clinical Excellence (NICE) guidelines for the use of anti-dementia drugs (2001).
Administers medication orally and by injection.
To advise on medication management issues.
Demonstrates a working knowledge of the key elements of the National Service Framework for Older Adults, the Community Care Act (1990), the Mental Health Act (1983).
To demonstrate knowledge about the existence of co-morbid physical pathology that may deteriorate the mental health of an older adult.
To work with up to date strategies for a range of mental health problems e.g. dementia, depression, dual diagnosis, thought disorders, drug and alcohol dependency requiring community detox programmes.
To work autonomously, using own time effectively to manage workload and the workload of others within the team.
Making use of evidence based strategies and psychosocial interventions in the care and treatment of people with mental health illness.
Enabling staff to use evidence based strategies and psychosocial interventions in the care and treatment of people with mental illness.
Act as a Clinical Supervisor and appraiser to an agreed number of staff.
To clinically manage other staff within the team.

To provide a source of information on how to best meet the mental health needs of adults for staff, service users and carers.
Regular provision of evidence based advice and training to staff and students including person centred approaches to dementia care (Kitwood 1990).

Evidence of positive attitudes and evidence based interventions reflected within nursing, medical documentation.
To contribute to the provision of an appropriate learning environment for pre and post registered students. This may include formal teaching sessions. Maintaining the learning environment through best practice standards, use of research and development.
Maintaining and up dating the learning environment of the Clinical Placement area requirements. Evaluating through student feedback, faculties of health and social care input and mentor self appraisal.

To receive clinical supervision in accordance with the Position statement on clinical supervision for nursing and health visiting (UKCC 1996) * on a contracted and regular basis from an identified Senior Nurse Practitioner in Mental Health or appropriate Practitioner of the individuals choice, as laid out in the Trusts Supervision Policy.
To be aware of the need for appropriate management support and supervision from the line manager.
To maintain health and safety in accordance with the Health & Safety at Work Act (1974).
To work to the Clinical & Personnel Policies and Procedures of AWP Trust.
To up date on mentorship responsibilities as expected by The Confederation, and in maintaining Clinical Placement Standards.
To work within Clinical Governance Guidelines as laid out in Clinical Policies of the Trust.
To maintain all statutory training e.g. basic life support, break away, fire safety.
To keep contemporaneous and accurate mental health records in accordance with AWP Trust policy Good Practice Guidelines for the Completion of Health & Social Care Records (AWP July 2000) and Guidelines for records and record keeping (UKCC 1998). To be responsible for the relevant section of the Single Notes process introduced by the Trust 2003.
To make appropriate use of time and unit resources; ensuring that record keeping and administration workload is achieved within working time.
To collect and input information (MHIS) and appropriate statistics for providing overall information about clinical activity, case load, clinical practice and about the post.
To participate in relevant research activity identified by the service.
To participate in relevant clinical audit identified by the service.
To keep up to date with national audit and research regarding older adults with mental health needs.
To work within the framework of research governance.
To initiate local evaluation of Older Adult services, in order to inform the development of future service planning
To participate in and contribute to the planning and development of services to meet the mental health needs of older adults by attending relevant meetings (e.g. Case Conferences, CPA reviews etc).
Providing accurate written reports as evidence of planning need.
Liaising with service users, carers and other voluntary and statutory organisations to determine service development need.
To assist the management process in the planning and review of services to meet the mental health needs of older adults.
To contribute to and have knowledge of the CMHT budget expenditure and provide service need information for the Community Care budget.
To contribute to clinical policy and clinical governance.
To ensure that clear care planning includes safe directive and prescriptive interventions to be used by the community support workers.
To review and supervise the role of the community support workers when they are involved with any service users on the case load.
To facilitate and manage any changes to care plan, communicating this with the community support workers.
To include community support workers written and verbal reports into the evaluation process of care planning, ensuring that notes are kept accurate and up to date.
Ensure that community support workers have contingency plans to work to in any unexpected change to a service users mental health state, particularly for out of hours working.
To make use of professional leadership skills throughout the service operations.
