Job summary

Come and join the North Somerset Recovery Team, based in Weston-Super-Mare. Our team is based just 5 mins from Jct 21 of the M5, the A370 and the A38. We are also lucky enough to be based within walking distance of Worle Train Station.

We are a friendly supportive team who are passionate about improving the lives of service users with complex mental health needs.

In this integrated health and social care team we focus on the delivery of recovery-based interventions, and self-management. We are a forward-thinking service and for many years have implemented a new way of working with service users with emotionally unstable/borderline personality disorders. This is an evidence-based model of care called Structured Clinical Management. You will be offered a three-day training package on how to deliver this model along with specialised individual and group supervision.

If you are a newly qualified nurse thinking about applying for this role you will be provided with an excellent preceptorship package, with support from a dedicated preceptorship Nurse and a supportive induction period. Within the Recovery Team we are experienced in supporting newly qualified staff to complete their Preceptorship and have supported team members to go on to get promotions within the team.

Main duties of the job

As a care co-ordinator in the team, you will be responsible for a caseload of service users with a range of complex mental health needs, working within the CPA framework to deliver a range of health and social care interventions.

You can expect to receive excellent supervision, and there will be opportunities for career development and training in order to support you in this role.

If you are a committed mental health professional with an RMN, Social Work or Occupational Therapy qualification, with excellent communication skills, who likes working as part of a team please apply.

Recovery practitioner will deliver a range of activities/defined interventions to improve the friends/relatives/carers (carers) ability to support the service user and to enable them in their relationship with the service user.

Recovery practitioner will develop and maintain good partnership working with other services is maintained throughout all treatment episodes, including regular liaison within Primary Health Care Team, inpatient services, day services, voluntary sector and with nominated carers/advocates.

Recovery practitioner will, in collaboration with service users and carers, to be responsible for facilitating the development of comprehensive crisis plans, rapid access plans, advance statements etc, involving other agencies such as primary care etc where appropriate.

Recovery practitioner will personally build, hope inspiring relationships with service users, which acknowledge the personal journey of each person, and focus on strengths and aspirations to allow the creation of meaningful personal recovery plans.

Recovery practitioner will be responsible for maintaining own workload on a day-to-day basis, ensuring that time is prioritised effectively, making full use of electronic resources such as diaries/scheduling.

Recovery practitioner will be responsible for the protection of individuals from abuse and harm in line with local safeguarding policies and procedures, by contributing with others to the plans to protect people at risk, ensuring the appropriate sharing of information.

Recovery practitioner will facilitate access for service users and carers to appropriate community services and interventions outside secondary mental health services and across the complete recovery pathway.

Recovery practitioner will work collaboratively and sensitively with individuals with a range of mental health needs to develop skills to manage their own health, in accordance with their personal recovery plan, by actively promoting and using approaches which are affirming, build on strengths, identify past positive experience and success, and use small steps to move towards the person's goal.

Recovery practitioner will maintain the single health and social care record, ensuring both paper and electronic records are kept up to date in accordance with professional and organisational standards.

Recovery practitioner will report and record within agreed timeframes, all activity relating to information reporting and performance requirements.

Recovery practitioner will proactively participate in management, workload and clinical supervision in accordance with trust policy, taking personal responsibility for making appropriate arrangements.

Recovery practitioner will provide mentoring/ training for others in relevant practice areas, according to professional requirements, taking a collaborative approach to practice development and evidence-based care.
