BSW Physical Health Care Service Lead - 8a
People with Severe and Enduring Mental Illness are dying 15-20 years younger due to avoidable physical health conditions. In Bath Swindon and Wiltshire (BSW) we are pleased to be in a position to offer you a key role in making a positive difference to the life and wellbeing of this cohort of people.
As the Lead, you will assist in developing and shaping the service offer to ensure people on the GP SMI register and under the care of AWP teams receive appropriate physical health care including the completion of relevant health care checks.
The postholder will provide Leadership to a new team of physical health clinicians working across the three counties, which form the BSW division. The post holder will Lead, this new service in partnership with a Consultant Psychiatrist and GP both with an area of special interest in relation to this service.
Main duties of the job
As the Lead, you will assist in developing and shaping the service offer to ensure people on the GP SMI register and under the care of AWP teams receive appropriate physical health care including the completion of relevant health care checks.
The Lead role will have responsibility for:
- Owning and leading the implementation of the physical health strategy across the Division, working closely with corporate and operational leads to ensure that the service provided meets national standards and local ambitions
- Be accountable for delivering physical health check standards and completion of all necessary data capture
- Oversee and coordinate training and development of not only the physical health team, but also engaging with wider community teams to ensure that all staff have essential training in identifying and understanding the physical health needs of their patients
- Ensuring that existing and new care planning arrangements (in line with the national position statement to move away from CPA) incorporate physical health monitoring
- In partnership with primary care, community teams and third sector colleagues, ensure that opportunities are created and taken to deliver integrated models of delivering physical and mental health care, e.g. links with Local Authority Health Trainer provision
