Job summary

An exciting opportunity has arisen for a full-time clinical pharmacist to join the Neurosciences Team at North Bristol NHS Trust. We are looking for an experienced and motivated pharmacist with excellent organisational and team working skills. Experience in the pharmaceutical care of neurosciences patients is desirable but not essential. You will be welcomed into a supportive team and develop expertise in the clinical area.

You will work alongside the Lead Neurosciences Pharmacist and be a key member of the multidisciplinary team delivering and promoting evidence-based, cost-effective and patient-focused pharmaceutical care to neurosciences patients.

Key activities will be to:

  • Ensure the safe provision of complex and high-cost medicines to our large cohort of outpatients with a range of neurological conditions (including multiple sclerosis, migraine, epilepsy, movement disorders and spinal muscular atrophy).
  • Provide specialist pharmaceutical care to neurosciences inpatients.
  • Support the education and training of pharmacy colleagues and other healthcare professionals in relation to neuro-pharmacy.
  • Apply pharmaceutical expertise to review, develop and implement guidelines and policy relating to the care of neurosciences patients.

Furthermore, you will support work to develop the neurosciences pharmacy service. Hence, this is an excellent opportunity for an ambitious pharmacist with an interest in service development.

Main duties of the job

  • To provide specialist clinical pharmacy services at a high level to the Neurosciences Specialty of the Neurosciences and Musculoskeletal Division.
  • To provide support to the highly specialist neurosciences pharmacist.
  • To teach and supervise less experienced pharmacists, technicians and students.
  • To act in accordance with Medicines legislation, the code of ethics of the General Pharmaceutical Council, United Kingdom Medicines Information (UKMI) standards and Trust policies and procedures.

The Institute of Neurosciences at North Bristol NHS Trust is the regional centre for Neurological services. It provides specialised services for a wide spectrum of Neurological conditions.

The service has an excellent national and international reputation, with patient referrals from across the South West, throughout the UK and worldwide.

Using the very latest cutting-edge techniques the Neurosciences team treat a wide range of conditions, Some examples include the use of pioneering deep brain stimulation techniques for the treatment of functional disorderssuch as Parkinson's disease, and developing the use of robotic surgery to deliver drugs directly into the brain with pinpoint accuracy.

We are a large pharmacy department with over 70 clinical pharmacists including many Specialist posts. The clinical ward pharmacy service is well established and provided by pharmacist and medicine management technicians. As part of the new hospital, we have a very modern pharmacy department including the largest dispensing robot in Europe.

Finance, service development, governance

  • In conjunction with the senior clinical pharmacists provide and develop a specialist pharmaceutical service to the Neurosciences speciality within the Trust.
  • Attend speciality meeting and meetings with Speciality and Divisional management teams as required and produce reports on activity as directed.
  • Advise on strategies to ensure cost-effective use of resources and be involved with achieving CIP targets. Provide feedback regarding this to prescribers.
  • Support achieving general pharmacy and appropriate specialist CQUIN targets.
  • To work closely with the Lead Formulary and Lead NICE pharmacists to help progress the Pharmacoeconomics element of the service.
  • Produce a monthly financial report for the directorate.
  • Record and investigate incidents in line with Trust procedures.
  • Record clinical Pharmacy activity in line with departmental procedures.
  • Audit and risk manage selected areas of practice to promote continuous quality improvement as required by the Division, Pharmacy department or the Trust.

Clinical

  • Clinically screen prescriptions to ensure the safe, efficient, evidence-based and rational use of medicines in line with the Trust Formulary and departmental policies so that risks to patients are minimised.
  • Challenge inappropriate prescribing and escalate as appropriate.
  • Support and promote antimicrobial stewardship
  • Be the named pharmacy contact for the clinical screening of homecare prescriptions for neurosciences outpatients.
  • Ensure that an accurate medication history and a full medicines reconciliation (in line with Trust policy) is completed for all patients seen as part of a clinical pharmacy service to allocated inpatient ward(s).
  • Provide advice and information to patients/carers to ensure compliance with complex treatment regimens including immune suppression and antiepileptic therapies
  • Provide accurate medicines information to other healthcare professionals and patients.
  • Provide support and implement the self-medication policy for patients able to manage their medications in this manner.
  • Deliver effective management of patient's medication to ensure timely and seamless discharge of patients. Communicate effectively with healthcare colleagues in Primary care when necessary as part of this process.
  • Maintain medicines management systems in conjunction with patients/carers, ward staff and pharmacy staff.

Education & Training

  • Contribute to the training of pharmacy undergraduates, medical students, pre-registration pharmacists, pharmacy technicians and medical and nursing staff as appropriate.
  • Support students undertaking modules or elements related to neurosciences as part of the Postgraduate Clinical Diploma
  • Train other pharmacists to provide cover to the neurosciences service when needed.
  • Support the development and training of trainee pharmacists as needed.
  • Keep up to date with developments in Pharmacy in order to maintain a high level of professional knowledge and competence. Attend study days and courses as necessary for the development of the service and the individual.
  • Maintain a record of CPD in line with GPhC requirements.

Leadership & Management

  • Represent specialist pharmaceutical interests in multidisciplinary meetings within the Trust and in the wider local health economy, sharing knowledge and experience with other neurosciences pharmacists at a local and national level.

  • Support management of the band 6 rotational pharmacist, including: ward visits, undertaking annual appraisal and ensuring they are meeting their agreed objectives
  • Supervise medicines management technicians and/or other dedicated pharmacy staff

  • Participate in the pharmacy weekend, bank holiday and late duty service.
