Job summary
We have an exciting opportunity for an organised, motivated and proactive individual to join our Rehabilitation Unit within Whitby Community Hospital, in the Coastal town of Whitby North Yorkshire.
You will be part of a supportive multi disciplinary team, seeing a range of patients in a beautiful area of Yorkshire.
You will be involved in patient assessment and treatment, working as part of an MDT to ensure patients receive appropriate support and skills to return to their home surroundings safely after hospital discharge despite complexities in health and wellbeing.
To succeed in this role you will need to be passionate about rehabilitation, to be able to deliver key priorities for complex patients with enthusiasm and have the tenacity to deliver high quality care in an ever changing environment.
Main duties of the job
- To provide care across the interdisciplinary team providing advice on managing the patients' functional ability and guide discharge planning.
- To carry out assessment and interventions for a range of conditions.
- Demonstrate leadership around individualised goal setting by allowing patients to work towards fulfilling their potential in a targeted and realistic way.
- To undertake a comprehensive assessment of patients including those with diverse or complex presentations and to use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
- To accept responsibility for a designated caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time.
- Regular lone working in the community visiting patients in their own homes.
