Job summary
We are seeking an enthusiastic, forward thinking, passionate, creative and clinically advanced Senior clinical lead. The post will work across the Older Peoples Planned Care Services in conjunction with unplanned services to solidify seamless care pathways for the people who access our services. This post is newly created and is reflective of the continued growth and development of Older Peoples Mental Health in Humber.
The Older Peoples Community Mental Health teams are multidisciplinary teams, that take referrals for older adults needing secondary care support with the their mental health. We take referrals From Hull and across the East riding and have 5 individual teams across the patch.
Older peoples service's are 2 years into a 5 year development project to ensure we can meet the needs of a changing population, by providing evidence based, trauma informed and effective mental health care. The post will play an integral role in the design, implementation and review of our services towards ensuring excellent and sustainable care.
Main duties of the job
o Hold a professional Mental Health Nursingo To work within the Older peoples planned services demonstrating and applying an expert level of clinical practice, leadership and management, education and research.o Work in conjunction with service manager and clinical leads / Team Leader's and be accountable for the delivery of high quality clinical services to patients.o Demonstrate critical thinking and analytical skills incorporating critical reflection in the clinical decision-making process.o Provide advanced level consultancy and advice in relation to patient care and to support the management of complex presentations.o To take a lead within older peoples planned care towards seamless care pathways between planned and unplanned services, ensuring collaborative approaches.o Lead on and support with the development of clinical policies, procedures and guidelines to support the delivery of safe and effective care and interventions.o To provide expert guidance relating to the provision of Mental Health services, including ensuring effective governance within areas of responsibility.o To work in partnership with local and regional agencies/ Voluntary Care Sector partners to deliver the continuous improvement of Mental Health services.o Participate in the Humber on-call manager system.o To deputise for the Divisional Clinical Lead as appropriate..

Essential
- Educated to Post Graduate Level or Degree plus significant advanced knowledge acquired through experience of relevant health interventions and additional specialist training.
- Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
- Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. mental health act, mental capacity Act,
- Registered under the NMC or HCPC
Desirable
- Member of organisational group/network, locally/regionally/nationally
- Advanced clinical qualification in CBT, DBT etc / Medication Management / Advanced Practice
Experience

- Evidence of leadership and organisational development issues that have had a positive impact on service delivery.
