Job summary

This is a rare and exciting opportunity for a highly skilled palliative care nurse to lead provision of excellent palliative care across Humber Teaching NHS Foundation Trust. The Trust provides services across Community, Primary Care & Addictions Services, Mental Health, Learning Disabilities and Children's Services.

We are seeking a Band 8a nurse to work 18.75hrs to provide professional leadership in palliative and end of life care across all our diverse services, recognising the shared and specialist needs in each setting to ensure equity of care provision. The post holder will be responsible for the continued implementation of our Palliative and End of Life Strategic Intent.

Our aim: All the people we care for who are nearing their end of life: live well until they die, have as good a death as possible in familiar surroundings, with the people they wish, and their family and friends are supported. We will do this in partnership and collaboration.

The opportunity to extend the hours to fulltime for 12 months is available to the successful candidate to deliver the Trust Palliative and End of Life Training Programme.

Main duties of the job

  • Have a local, regional, and national profile in Palliative Care and End of Life
  • Promote and embed a proactive approach to End-of-life care in Humber Teaching NHS Foundation Trust and a proactive approach to recognition of End of Life.
  • Responsible for the co-ordination of education of Palliative and End of Life care in Humber Teaching NHS Foundation Trust
  • Work as an autonomous practitioner and link as a resource of advanced knowledge, expertise in the field of Palliative and End of Life care within Humber Teaching NHS Foundation Trust.
  • Act as a clinical resource utilising specialist knowledge, supporting staff education and development across the Trust to enhance quality care at End of Life.
  • Identify and monitor closely clinical outcomes achieved across the Trust and take steps to improve clinical outcomes.
  • Promote clinical excellence by undertaking clinical audit and evaluation to advance the knowledge and expertise at team and Trust level
  • Work in partnership with operational managers, clinical leads and other colleagues.
  • Continue to demonstrate advanced clinical skills in practice, providing support and advice re. clinical developments, delivery and co-ordination of the highest quality care in collaboration with patients, carers, staff, managers, other agencies, appropriate to the service area
  • Maintains compliance with, and development of, clinical policies, procedures and guidelines.
  • Co-produce and facilitate ongoing delivery of Trustwide End-of-Life Strategy for HTFT.
