Job summary

The Community Forensic Teams are undergoing service transformation and an exciting opportunity has arisen for an Advanced Forensic Practitioner to provide clinical leadership in the mental illness and personality disorder forensic pathway. You will be a registered mental health professional with significant forensic experience. You will clinically lead and develop other professionals in the team whilst managing a caseload in the community. The post holder will provide specialist consultation and advice to non-forensic services and support forensic inpatient services in determining safe discharge pathways for service users.

The post presents as an ideal opportunity for someone who holds a keen interest in leadership with a desire to champion a good quality service.

The successful candidate will provide strong and effective leadership to the Teams working closely with the MDT to provide high quality clinical provision. You will promote the vision and values of Humber NHS Teaching Foundation Trust. The community services are a fast-developing area of provision within the Forensic Division. The successful candidate will be a talented, dynamic and dedicated leader with a passion for delivering a high quality service.

Main duties of the job

To provide direct clinical care, ensuring clinical systems are in place to monitor and support practice and performance.

To provide therapeutic intervention and specialist assessment as part of a multi-disciplinary team.

Act autonomously and in conjunction with the wider MDT in Assessing, diagnosing, planning, and implementing care.

To provide clinical advice and leadership providing profession specific advice and leadership to the wider team

To work with service users and carers to facilitate a collaborative approach to engagement in individual care and service development

Understand and ensure patient safety, risk reduction/mitigation and competent practice in self and others.

Initiate, evaluate and modify a range of interventions tailored specifically to the individual. This will include delivering therapies and evidence-based interventions

Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and support staff to do likewise to ensure safety of individuals, families, and carers

Support training programmes within the service and to support stakeholder training events

To provide advanced clinical expertise in consultancy

Ensure robust processes are in place to support and debrief staff after incidents and that learning from incidents is applied within the service.
