Job summary
We are currently looking to recruit an enthusiastic and motivated Band 4 assistant practitioner to join the therapy team 22.5 hrs per week. You will be an integral member of our multi-disciplinary community based team working in Pocklington. You will be expected to deliver clinical care delegated by a registered professional and contributing to the assessment and evaluation of person centre care plans.
The role requires excellent communication skills, IT skills, and the ability to be adaptable, reliable and highly organised. Already established relevant community clinical competencies are desirable for this post.

Qualifications and Knowledge
Essential
- oRange of work procedures and practices, majority non-routine; intermediate level theoretical knowledge. Diploma or equivalent appropriate qualification, e.g. foundation degree; or NVQ3 level qualification plus short courses or relevant experience to diploma level
- oGCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
- oHealthcare qualification
- oKnowledge of a range rehabilitation techniques / community services and urgent crisis response.
- A good understanding of rehabilitation and community services
- Understanding of the importance of following procedures and treatment plans
- Knowledge of basic assessments, including risk and the ability to identify needs and care planning

- oEvidence of continued professional development documented through Trust PDR/KSF/Supervision processes and personal portfolio
- oUnderstanding or application of relevant clinical practice/standards/audit within identified clinical area
