Job summary

An exciting opportunity has arisen within the YOURhealth senior team to support our North Yorkshire Weight Management programme in North Yorkshire areas of Scarborough and Ryedale.

You will be required to support and drive forward completion of 12 and 24 week weight management programmes, engage with stakeholders, drive through new initiatives and support the YOURhealth team to reach their annual targets.

Main duties of the job

The main duties of the role include -

  • Provide day to support and line management for the Weight Management Advisors
  • Be the main contact for core stakeholders
  • Review and analyse data to ensure the service is on track to reach the required targets
  • Contributes to, and advises on, the development and implementation of local health improvement programmes.
  • Develops and maintains public health information and support structures
  • May maintain information directories, data collection system
  • Identifying and engaging with clients from agreed target groups.
  • Supporting clients to identify their health needs and what barriers prevent them from making healthy choices.
  • Encouraging clients to increase their ability to make better health choices and supporting them in initiating and sustaining appropriate behavioural changes using a mutually agreed Personal Health Plan.
  • Recording and reporting activity and results.
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload.
  • Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
  • Participate in regular supervision and continual learning.

Essential

  • Project Management qualification such as PRINCE2 or diploma equivalent or equivalent experience
  • An understanding of the biopsychosocial model of health and the social determinants of health
  • Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as action planning, goal setting, problem solving
  • Excellent group and one-to-one facilitation skills including conflict resolution
  • Proficient in Microsoft Office and web-based services
  • Commitment to continuous learning and development.

Desirable

  • Knowledge/experience of prevention
  • Possession of general education qualification in English i.e. GCSE or equivalent

Experience

  • Experience in utilisation of clinical systems such as SystmOne and PharmOutcomes
  • Prior experience in supporting a project
  • Experience of working in a team
  • Understanding of health promotion and behaviour change
  • Managing a caseload and daily dairy
  • Supervisions

  • Experience of working in health and social care care/community development setting or similar Networking experience
