Main duties of the job
Your role will centre on the delivery of specialist mental health support to people with severe and enduring mental illness who are requiring treatment and care in the community.
This will be in the form of direct work and care coordination with people on caseload, providing duty support to those in the team and contributing to multi-disciplinary team meetings both in house and with colleagues externally.
Empowering and supporting service users to make effective personal decisions which promote their psychological well being and recovery.
Undertakes supported person led assessments, risk assessments, support planning and outcome focused reviews of service users and carers who have eligible needs and complex and/or long term conditions in accordance with legislative requirements, policy and professional frameworks.
In addition to undertaking person led assessments and operational responsibilities will manage a caseload with complex needs and provide case load guidance, support and specialist advice to junior members of staff and ensure care in completed within the practice and performance framework.
Key relationships are with: Clinical and Team lead within the CMHT for immediate day to day support and management.
Due to working in the Haltemprice area and surrounding villages, it's required that applicants can commute effectively across these geographical areas.
