Job summary
We are looking to recruit a Health Education England (HEE) Social Prescriber to deliver remote Social prescribing for Yorkshire and Humber, North Cumbria and North East Of England HEE. The geographical location may change but as this is a remote offer you will provide the support continuously online.
This will be delivery to:

- International medical graduates within the GP school for NC and NE
- International medical graduates within all medical specialisms for Yorks and Humber (GP, psychiatry, medicine, pathology etc)
Social Prescribers give one-to-one and group support, help people to overcome barriers to improve their lifestyle and provide on-going encouragement and motivation. Due to the geographical range of the role the support provided will be via remote/virtual offer to the individual.
You will have a sound understanding of the issues affecting isolated and vulnerable people, the ability to motivate people to make changes and have excellent communication skills working within genera practice and supporting the primary care model of delivery to identify and work closes with patients, supporting them to make positive choices that will improve their overall health and wellbeing.
Main duties of the job
You will also be expected to promote and market healthy lifestyle issues within the local communities to encourage, change within areas of deprivation and isolation.Excellent interpersonal communication skills are an essential requirement of Social Prescribers as they will be working with individuals (and occasionally groups of people) from a variety of backgrounds and have a sound understanding of behaviour change techniques and asset based community development.
The post holder must be flexible to work daytime and evenings, including weekends. You must also be required to commute across the geographical boundaries of East Yorkshire and York should this be required for meetings, training etc.
