Job summary
Humber Teaching NHS Foundation Trust are 1 of only 11 National pilot sites across the country to test new ways of delivering community mental health care.
As part of this exciting development we are seeking to appoint a Medicines Optimisation Technician to join our team.You will be working within multi-disciplinary teams including; Pharmacists, Medicines Optimisation Technicians, Senior Mental Health Nurses, Social Workers, Psychologists, Psychology Assistants, Peer Support Workers and Mental Health and Wellbeing Coaches.
We are looking for a motivated and experienced technician to join our team to help shape and improve the delivery of contemporary community mental health support and treatment.
Hours of work are flexible. Typical working week will be Monday - Friday, 09.00 - 17.00 excluding bank holidays and weekends.
Main duties of the job
You will support the lead pharmacist to deliver this new model of care and be a vital link between the community mental health teams and the GP surgeries. The purpose of this role is to lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care.
The post holder will help patients to get the best from their medicines through effective medicines reconciliation after discharge and on other transfers of care, medication use reviews, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence.
The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within community mental health teams and general practice.
In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments.
The successful candidate will be registered as a Pharmacy Technician with the GPhC . You should be friendly with excellent time management and prioritisation skills. You should be able to problem-solve and take responsibility for organising your daily workload in liaison with the line manager.
