Job summary
We are looking for a band 5 occupational therapist to join the mental health crisis and intervention team specifically within the home based treatment service based at Miranda house, inpatient assessment unit and the homeless mental health team.
We are looking for individuals with interest in working with people who have mental health problems and complex needs. We want people who can adapt to changing environments and who can be an active member of the MDT promoting occupational therapy.
The Occupational therapy role focuses on assessment of needs and have a role in identifying patient pathways. The team works across a large geographical patch with a mixture of community and home visits with some flexibility required. The OT service is guided by MOHO model of practice with opprotunity to be involved in developing intervention plans carried out by associate practitioners.
All Occupational Therapists have access to supervision, CPD networks, and newly qualified OT's are invited to participate in a monthly Band 5 OT Development Group and preceptorship programme.

Main duties of the job
The post-holders will be expected to assess patient needs and deliver, evidence-based OT interventions, to facilitate recovery/wellbeing, and optimise independent functioning.
The post will be involved in the referral decisions and prioritisation of OT input alongside other OT colleagues. The post will need to work autonomously and make clinical decisions based on risk and patient presentation with supervision.
There will be an expectation to be involved in patient pathway planning and handover OT intervention plans to teams continuing their care.
The post will make professional links with community provisions, 3rd sector organisations and with other NHS teams to ensure continuity of care and access to appropriate services in the patients local community.
Interventions take place in home environments, community settings and in-patient units, with flexibility of working required to meet service user needs.
