This is an opportunity to become part of the innovative Memory Assessment Service. There are 45 hours available and applications will be accepted from those who wish to work both full and part time hours. Please state in the supporting information how many hours would be preferred.
Applications from Registered Nurses of any Specialism are welcome if they have relevant knowledge, experience and interest in working with those living with dementia.
The Team offers comprehensive Memory Assessment and, for those receiving a diagnosis of Dementia, treatment and/or ongoing interventions.
MAS works in partnership with the Alzheimer's Society and Carer's Support Services. They also work alongside a network of many agencies who support those living with Dementia to ensure they are able to live well with their diagnosis.
We are looking for Band 5 Nurses to work predominantly in Hull but with the expectation that there may be occasion when they are expected to work in the East Riding of Yorkshire . The team works across many venues to offer local assessment to those referred. The ability to travel across the whole of the East Riding is essential. Some clinic venues are not easily accessed by public transport therefore the need to have access to transport is necessary.
The team provides a positive, forward thinking environment where quality is paramount in the delivery of a Memory Assessment, Diagnosis and Post Diagnosis Service. Team and personal development are ongoing.
Main duties of the job
To carry out cognitive assessment, assessment of need, have an awareness of carer need, presentation of assessment information for formulation, providing information to patients and their carers regarding the outcome of assessment. To contribute to the Multi Disciplinary Team. To provide supervision to other members of the team.
The successful candidate will also be involved in other aspects of the service to include information giving events and being part of service developments.
