Job summary
We are seeking to recruit an experienced and driven professional to join our - Single Point of Access (SPA). The SPA is a developing and innovative service providing the day-to-day management of referrals for the Humber and North Yorkshire Provider Collaborative.
We are seeking to recruit a candidate who can support the next phase of growth within the SPA, with the development and operationalisation of an integrated inpatient and community model (including, Learning Disability Forensic Outreach and Liaison Services and Specialist Community Forensic Teams).
Main duties of the job
The post holder will be required to assist the SPA Lead in the planning and delivery of an inpatient and community Single Point of Access (SPA) for the Humber and North Yorkshire Specialist Provider Collaborative.
Candidates will be required to provide specialist forensic skills in the assessment, triage and coordination of referrals for specified services. The SPA Coordinator will be a highly skilled clinician offering and advanced perspective in the processing of referrals and will support operational colleagues and managers in the operational delivery of the SPA.
Candidates must be highly organised and able to work to tight deadlines, whilst maintaining accuracy and attention to detail. Excellent communication skills are essential with the ability to convey complex information to a range of stakeholders.
