The East Riding Partnership is an established formal partnership between Humber NHS Teaching Foundation Trust, The Alcohol and Drug Service (ADS) and NACRO.
An exciting opportunity has arisen within the addictions service - We are seeking a Fulltime permanent RNMH/RGN to work within the field of addictions as a Team Leader/ complex case manager, focusing on service users who have substance misuse problems Alcohol/Illicit substance.
You will work clinically within all areas of the substance misuse treatment delivery system and role model good practice to all staff. Care planning and review, medication titration and dispensing and focusing on a patient's long-term recovery.
You will supervise a core group of staff, develop the staff group through the appraisal process and mirror good, safe, innovative practice. You will be instrumental in DATIX reporting/ action planning .
Main duties of the job
The role predominately involves the Nurse working within the East Riding Partnership Hubs and supporting other services and agencies to enable recovery of this high-risk patient cohort - based on factors such as high doses of opiates, length of time prescribed, patients with a previous diagnosis of substance misuse or mental health problems, or those with new presentations.
You will carry a case load of complex clients and manage these client through/ ongoing recovery.
You will provide specialist advice on substance misuse and related issues to a range of health and non-health professionals across the service, related to the assessment, planning, implementation and evaluation of the plan of care for patients referred to East Riding Partnership. This includes advice on treatment options including detoxification, substitute maintenance prescribing, symptomatic management, the management of withdrawals and detoxification, Local and National guidance, managing all aspects of clinical safety, e.g., potential overdose, withdrawal symptoms and complex presentation, with possible high co-morbidity risk factors.
The post holder will assist the senior management and leadership team to lead on practice development, clinical governance, audit and standards within the service.
The post holder will support key performance data collections and the demands placed on the service area to inform , support service development , provision and present this data to our Commissioners.
