Six Month Secondment or Fixed Term Opportunity- Learning Disabilities Liaison Nurse

An exciting opportunity has arisen for an enthusiastic, well-motivated and knowledgeable practitioner to work within and be based within the Safeguarding Team in Hull University Teaching Hospitals as a Learning Disabilities/Autism Liaison Nurse.

The post holder will be expected to support patients in the acute trust who have a learning disability and or autism during their hospital journey. This will include Pre admission, inpatient stay and discharge planning including outpatient attendance, where it is identified that the patient and or carers will require additional support / reasonable adjustments.

The successful applicant will be supported in the post by the HUTH Safeguarding team and the Humber Teaching NHS Trust Learning Disability Services. There will continue to be close collaboration between these two organisations to ensure professional supervision and the management of the day to day duties of the role within the acute trust.

The role will enable the development of management, professional, communication and analytical skills for the post holder. There will be opportunities to work with many multidisciplinary/ multi agency teams both in the acute service and in community.

Main duties of the job

  • To ensure that people with learning disabilities and or autism have full and appropriate access to healthcare within the acute setting and to liaise with all relevant parties to obtain information and plan effectively for all hospital visits and to ensure effective discharge planning.
  • To ensure that systems and processes are in place to identify patients with learning disabilities and or autism and that their pathways of care are reasonably adjusted to meet their needs and include families and carers.
  • To ensure the availability of a comprehensive range of patient information, designed for and with patients with learning disabilities and or autism.
  • To play a key role in adult safeguarding cases involving a person with a learning disability and or autism accessing healthcare at the Trust.
  • To continue to develop and deliver effective training for Trust staff in relation to learning disabilities and or autism.
  • To provide expert advice and support to staff members with mental capacity assessment and Deprivation of Liberty Safeguards for people with learning disabilities and or autism.
  • To support and advise other professionals on issues related to learning disabilities, including participating and or leading in strategy decisions/meetings/case conferences.
