Job summary

An exciting opportunity has arisen for a dynamic physiotherapist to work 22.5 hrs per week that is looking for a challenge to join the Neighbourhood Care Service in Pocklington . You will work in an integrated multidisciplinary team working closely with social care and GP colleagues across traditional care boundaries in the integrated delivery of individualised, safe and effective, high quality care. You will want to work within a cycle of continuous improvement and will be interested in understanding the experiences of our patients and their families.

You will be encouraged to develop expert skills in the specific area in order to take a lead role in clinical practice issues, raising the profile of the service and offering specialist advice to planning and service development teams in order to assist in the implementation of service improvements including the Rapids Service and Hub beds (step down beds).

Experience of managing staff and a working knowledge of System 1 would be a distinct advantage.

You will be a qualified and registered physiotherapist with a history of relevant recent and ongoing clinical professional development and you will have a sense of value for inter-professional and work based learning. You will have excellent communication skills and will demonstrate positive leadership and role modelling.

You will be supported by your therapy team consisting of physiotherapist, OT, support workers, complex care nurse and the district nursing team.

Main duties of the job

To be accountable in own area of clinical responsibility, ensuring the delivery of evidence based clinical interventions and practice for a specialist caseload. To continue to demonstrate specialist clinical skills in practice and provide support, training and advice re the delivery of care in partnership with patients, relatives, staff, managers, and externally with other agencies. Provide specialist therapy advice and information, communicating with a range of people eg senior, middle managers, other clinicians within the organisation and other external agencies Regularly demonstrate specialist skills in assessing the needs of patients and their relatives, based on the analysis of a range of complex facts and situations. Interventions will be planned on the assessment and interpretation of this information Obtains, analyses and interprets assessment information, presenting symptoms and physical health to develop the appropriate intervention plan for a complex caseload
