Job summary

Are you an experienced registered nurse with an interest in Diabetes, looking for a challenge and desire to develop new skills and knowledge? Driven by the desire to improve patient care and service delivery standards?

We are looking to recruit an experienced registered nurse into a developmental role, commencing at band 6 rising to band 7 following 12 month development period. The successful applicant will be joining established multidisciplinary community teams in the Scarborough and Ryedale locality. The role will support service development and leadership within the Diabetes service.

Main duties of the job

  • To support the delivery of a Community Diabetes Service that embraces the recommendations of the National Service Framework for Diabetes
  • To be responsible for the management of a caseload of patients with complex diabetes problems who require specialist assessment and treatment. We provide care and support for secondary and primary diabetes community service, includes supporting the diabetes dieticians and diabetes consultants
  • Delivering educational programmes and clinical support to health and social care
  • To work to deliver service change and improvement and establish the direction of the diabetes service.

  • Degree level qualification

Desirable

  • Teaching educating qualification
  • Prescribing qualification
  • Diabetes qualification
  • Educated to Masters Level (e.g. MSC in Professional Health Studies) or Masters level/Post Graduate modules ( e.g Long Term Conditions) plus significant relevant knowledge/ experience in complex long term condition case management

  • Experience of Diabetes management in a community setting
  • Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care
  • Evidence of advanced practice skills and able to demonstrate the impact of this on practice change/development

Desirable

  • Band 6 registered nurse minimum 2 years experience
  • oAbility to work across team/organisational boundaries developing and maintaining multi-professional and multi-agency partnerships

  • Specialist understanding of relevant clinical practice, standards and protocols within identified clinical area.
  • Evidence of undertaking audits/surveys

  • Member of specialist interest group/network, locally or regionally
