EDS Pathway Coordinator

Job summary

An exciting opportunity has arisen for a Pathway Coordinator to join the highly specialised Ehlers-Danlos syndromes service covering the South of England and Wales in contributing to delivering an excellent service for people with, or at-risk of, rare, monogenic EDS types. The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, patient focussed and knowledgeable point of contact. To provide a consistent approach across the Trust, Standard Operating Procedures (SOPs) will describe how the functions of this role will be carried out in each service. The standard job summary is described below:

oTo be the key contact for the allocated grouping of clinical staff, services for all patient related and administrative matters.

oTo be the key contact for all patients under the allocated service.

oTo work within the Clinical Support Team to deliver high quality administrative services to allocated clinical team.

oTo monitor patient's pathway, from referral, escalating issues promptly, in order to ensure compliance with waiting time standards.

oAs required, support the coordination of national and local audits by effective data collection.

Main duties of the job

To support the EDS lead and EDS coordinator to ensure all administrative process are delivered to a high professional standard.

To register new referrals and upload to Rego. To send out first contact letters or rejection letters.

To work with the electronic Patient System Cerner and register patients, upload reports and family trees on Phenotips.

Take responsibility for the efficient and professional handling of all telephone and email queries.

As required, provide administrative support to the allocated services.

Under the instruction of the EDS lead/ EDS coordinator, to cancel / reduce clinics as leave is booked. To be responsible for ensuring that clinics are booked to capacity, but appropriately for the number of clinicians.

To be responsive and to re-prioritise the patients to meet their clinical urgency and to make ad hoc urgent appointment as required.

To be aware of the patients waiting times for allocated services by ensuring that patients are re-booked and seen both within waiting time targets and as necessary for clinical urgency, and to escalate to the EDS lead or EDS coordinator if this is not possible.
