Paediatric Senior Staff Nurse (PATCH)

Job summary

London Northwest University Hospital Trust is seeking an experienced dynamic children's nurse, for an exciting paediatric hospital at home (PATCH) service. We are developing same day emergency care pathways to prevent and reduce hospital admissions. We will offer you a wide range of experiences, assessing and treating children and young people with acute illness in their own homes.

It is essential that you will have at least 2 years of experience working as a registered children's nurse, with excellent clinical and communication skills.

The Band 6 role requires an experienced, dynamic children's nurse, ideally with community experience working at enhanced or advanced level. You will need to be flexible and forward thinking, able to work independently and as part of a friendly and supportive team delivering nursing care to a diverse community. There will be a strong focus to deliver an 'out of hospital' agenda across community-based pathways to reduce hospital attendances and length of stay.

The PATCH team will cover a 7 day service between 08:00-22:00, the PATCH service has a flexible work bases at Northwick Park Hospital and Ealing Hospital, however if required to work on the Paediatric Assessment Unit for a fixed length of time, then this is a 24 hour service requiring flexible working.

Main duties of the job

KEY RESPONSIBILITIES

Working relationships and communication requirements of your job

To communicate effectively with staff and patients to ensure that the highest possible standard of care is provided to children and their families and that comprehensive is documented.

Accurate recording of clinical signs and interpretation of same in relation to the child's clinical condition.

Report to medical and or nursing staff on the condition and progress of patients

To plan, with the family, at the earliest possible opportunity, the discharge of the child liaising closely with all members of the primary health care team.

To ensure the confidentiality of patient information at all times.

To ensure the privacy and dignity of patients is paramount and not compromised

To communicate effectively with staff and patients to ensure that the highest possible standard of care is provided to children and their families and that comprehensive information is documented.

To respect the individuality, values cultural and religious diversity of patients and their families and contribute to a service provision sensitive to those needs

To participate in ward teaching programmes e.g. informal bedside teaching
