Job summary

We are looking for a dynamic, industrious and flexible individual to lead a progressive and forward thinking team that shares a great passion and vision in improving care for patient with diabetes across North West London. The successful applicant will be linked with the North West London Multidisciplinary Diabetes Foot Team which was established as part of the NW London Transformation programme for the Diabetic Foot.

The MDFT is part of the NW London Diabetes Foot Network which brings together multidisciplinary foot teams across NW London to share learning and promote best practice in the management of the diabetic foot. You will be part of a team that provides inpatient and outpatient podiatry care, training and education across our acute sites. The role will include excellent opportunities to work autonomously as well as across teams and disciplines to provide a high impact input into the outcomes of Diabetic Foot Programme in this sector; as well as contributing to building new service pathways.

The post-holder will work peripatetically across Northwick Park Hospital, Ealing Hospital and Central Middlesex Hospital.

Main duties of the job

To be part of an integrated LWNH Podiatry service which comprises a highly motivated team working where members are able to work autonomously and independently, providing and promoting excellent standards of high risk foot care working with multi-disciplinary teams in either an acute or community setting.

To work closely with the Acute Therapy Lead in providing effective leadership across the service combining service and operational management, as well as taking forward work towards a number of priorities/projects including clinical and service management support.

To line manage both clinical and non -clinical staff within operational remit

The post holder will provide specialist foot care for patients with diabetes/vascular conditions who have complex, high risk foot problems. This includes diabetes patients with acute and chronic foot wounds, patients with neuropathic and vascular complications. The post holder will be clinically responsible for treatment of the high risk caseload working autonomously as well as work as part of the multidisciplinary team.

The post holder will be responsible for the planning and contributing to the education and up-skilling of clinical and care staff, working with diabetic patients with diabetes and high risk foot conditions across the organization . There will be a responsibility for structured patient education for people with diabetes and their carers too.
