Endoscopy Co-ordinator Complex Polyp and Tertiary

Job summary

  • Responsible for co-ordination of care of patients referred through Complex Polyp and tertiary endoscopy Service.
  • Responsibility for daily liaison with Complex Polyp CNS and the consultants to ensure elective complex endoscopy (ESD/EMR) sessions are consistently booked with maximum utilisation, taking into consideration both clinical urgency and time on the waiting list.
  • Be responsible for monitoring / auditing trust data received from the Performance / Information department and managing waiting lists to ensure no patients breach the national access, booking and choice targets, working in line with the Trusts' Patient Access Policy.
  • Responsible for booking pre-assessment and associated appointments in a timely manner and adjust lists as required if any patients fail pre-assessment.
  • Liaise closely with Bed Management Teams, Wards, Pre-operative Assessment nurses, senior management and clinicians, PAS team and Theatre Co-ordinators and management.
  • To liaise with Endoscopy to be aware of any patients cancelled on the day or who do not attend and to take the appropriate action to rebook or discharge.
  • Effectively use digital platforms to streamline pre-assessment and scheduling pathways

Main duties of the job

  • Booking & Scheduling

    • The post-holder will be responsible for ensuring that elective complex polyp endoscopy (ESD/EMR) lists are booked.
    • Contact patients offering them a choice of admission dates and agree with them the date of admission in line with the Patient Access Policy.
    • Create a waiting list entry for patients requiring admission.
    • To ensure correct OPC code is used for waiting list entries.
    • Schedule patient appointments ensuring patients are fully booked on CERNER and Endo vault and ensure a confirmation letter is sent or that patients are telephoned if booking at short notice.
    • Responsible for booking patients in chronological order or in order of clinical urgency.
    • Lead responsibility for booking procedures ensuring that the endoscopy capacity is proactively and efficiently utilised.
    • Attend and action outcome of weekly Polyp MDM.
    • Book new appointments for Pre-assessment (POA) at least two weeks before endoscopy procedure, ensuring that capacity is proactively and efficiently used and patients booked in advance of endoscopy procedure.
    • Ensure that any instructions that have been communicated from the pre-assessment and Complex Polyp CNS are actioned.
    • Re-book patients in line with the 28 day cancellation breach date guidelines if cancelled on the day by hospital.
    • Liaison with other specialties for the scheduling of complex multi surgeon cases

  • Lead responsibility to identify and report any issues to the relevant manager which compromise delivery of the 18.week RTT pathway, e.g. lack of capacity either for endoscopy or POA.
  • To understand cancer and diagnostics targets and how it relates to scheduling of patients.

Communication

  • Develop key relationships with staff both within the local teams but also in the wider organisation to ensure the patient pathway is as swift and effective as possible in relation to the clinical needs, while delivering an exceptional patient experience.
  • Liaise with internal and external colleagues to share patient pathway information and diagnostic information, and expedite patient journeys where needed.
  • To deal with telephone enquires in an empathetic and productive manner, taking appropriate action to ensure high patient satisfaction.
  • Support the investigation of patient DNAs and deferrals, communicating the results to the Consultants and service as appropriate and updating waiting lists accordingly.
  • To communicate details of any cancellations the Clinician and relevant service ensuring that all information is available.

Data Quality

  • Ensure trust systems are updated with patient pathway status information and that data quality is maintained.

  • Use Trust IT systems and databases to ensure relevant and appropriate patient information is entered and captured. Input data onto iCS in accordance with departmental policy and procedure.

Service Delivery and Reporting

  • Support the identification and reporting of any issues to the Team Leader which compromise delivery of the 18 week RTT pathway, e.g. lack of capacity for surgery
  • Ensure that Trust agreed standard of service are delivered to patients requiring elective admission to the speciality.
  • Provide support during absence of other staff within the team.
  • Work to strict timescales and be able to prioritise workload to best effect.
