Job summary

Are you an experienced Diabetes Specialist Nurse looking for an exciting opportunity to develop your skills and implement new and innovative ways to deliver diabetes care?

We are looking to recruit a skilled and dynamic team member to join the diabetes nursing team at London North West University Hospital NHS Trust.

Due to an expansion of the team and implementation of new ways of working to include virtual wards we have this great opportunity.

Main duties of the job

To act as a role model and expert practitioner in relation to diabetes within the inpatient, outpatient, and virtual ward setting.

To have lead nursing responsibility for a caseload of patients with diabetes.

To co-ordinate all aspects of nursing care and support for people living with diabetes in their social, emotional and health needs, referring to appropriate professionals and services where necessary.

Act as an advocate for the patient through effective support and information enabling them to make choices and decisions concerning treatment.

To act as a resource to people living with diabetes and for other professionals caring for them.

To promote an innovative and progressive attitude to the continual improvement of patient care through research and evidence-based practice
