Job summary

We have two exciting positions for either a community CNS or a paramedic to join the team at Meadow House hospice. The community team provides a specialist palliative care service for the residents of Ealing and Hounslow. The service also has a 15 bedded in-patient unit, psychology, psychosocial, allied health professionals, a bereavement and lymphoedema service. So you would be joining a truly multi-disciplinary team.

Patients will present with complex and specialist needs and the focus of their care will have changed from curative to palliative.

Main duties of the job

The post-holders will have a strong clinical role evaluating individual patient needs including a holistic assessment and managing a patient caseload. The post-holders will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role. Key working relationships with primary care teams are essential using excellent communication and interpersonal skills, alongside a dynamic and motivated attitude.

We require someone who can work both autonomously and as part of a team. Leadership, innovation, motivation and excellent communication skills are essential. You must have either an RN level 1 or HSPC qualification with appropriate post registration qualifications and experience. You should have a first degree or be working towards this qualification and be committed to undertaking further study to meet service need.
