Job summary

An exciting opportunity has arisen for a paramedic with experience in specialist palliative and end of life care to join the multi-professional Ealing and Hounslow Community Specialist Palliative Care team. This will be a new role within the team which will offer the exciting opportunity to develop and enhance the service we deliver.

Main duties of the job

  • To provide an evidence-based Specialist Palliative Care Service to adult residents of Ealing and Hounslow CCGs that is equitable according to need. This service covers regular working hours as well as weekends and bank holidays.
  • To work collaboratively with the Primary Care Team, acute sector, social services and other agencies, to establish a close working relationship that is essential in ensuring that care is coordinated and responsive to individual need.
  • To manage a complex caseload, providing an advanced level of clinical expertise, ensuring it reflects the core components of the Paramedic role.
  • The post-holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role.
  • To act as a resource and educator to the primary care team and generic palliative care providers, helping to develop and empower their practice.
  • To demonstrate empathy with patients and carers, ensuring that effective communication is achieved particularly in circumstances where there may be high levels of distress.

To assess referrals from members of the multi-professional team to ensure they meet the criteria for specialist palliative care in an equitable way according to need.

To work to an advanced level, take the lead role in assessing, managing

and monitoring the physical, psychological social and spiritual needs of patients. This will include utilising well-developed skills in symptom management that are underpinned by evidence-based practice.

To advise and support the Primary Care Teams to enable them to provide an effective palliative care service, this would include contributing to advice on:

  • Pain and symptom control
  • Social, psychological and spiritual care
  • Identify complex problems for other professionals using advanced levels of judgment and decision making in clinical care
