Job summary
Applications are invited for a Speciality Doctor in Palliative Medicine to join the team at London North West University Trust which provides palliative and supportive services at Northwick Park Hospital, Central Middlesex Hospital, Ealing Hospital and the communities living in the London Boroughs of Harrow, Ealing, North Brent (and Hounslow). The service as a whole is entirely NHS funded, and there are very close links with the specialist palliative care service based at St Luke's Hospice, Kenton Grange.
Main duties of the job
This is a 7 session 3-month fixed term post based at Northwick Park Hospital.
The aim of the post is to.
- Provide specialist palliative medicine expertise to patients and the palliative care team at LNWUHT (mainly on the Northwick Park but occasionally on the Central Middlesex Hospital site), both directly and through indirect discussion.
- Play a role in improving the continuity of palliative care for patients during transitions of care between the community, Northwick Park Hospital and St Luke's Hospice.
- Work with the current palliative care team to continuously improve the skills of Trust staff of all disciplines and experience in care of the dying within LNWUHT.
- To contribute to the palliative care team's education and training programme for healthcare professionals on palliative care topics.
- To contribute to the development of the palliative care service at LNWUHT.
Close involvement with the end-of-life agenda, development and enhancement of links with local primary care teams and a commitment to education and service improvement will all be required in this role.

The post holder will work as part of the existing multidisciplinaryspecialist palliative care team in Northwick Park Hospital and willcontribute to the care of patients referred to this team as allocated on adaily basis.
Within the acute hospital, the post holder will together with otherpalliative care colleagues, provide clinical advice to ward staff caringfor patients requiring palliative care input at any stage of their illness.This will include a holistic assessment of the needs of patients andthose close to them, advice on management of symptoms and otherconcerns, and advice on appropriate transfer to other settings (e.g.home, nursing home, hospice).

- At least 6 months experience in palliative care +/or general practice
Knowledge and Experience

- Minimum 3 years post registration experience
- Minimum 6 months experience working in the community setting or general practice+/or 6 months experience in palliative care
- Demonstrates up to date clinical knowledge in palliative care
- Experience of multi-agency working
- Experience of implementing evidence based practice
Desirable
- Published Quality Improvement report or Research
- Previous oncology or palliative care experience
