Looking for a Community role in Adult Mental Health, to use your OT skill set?
We have an exciting opportunity for a Band 5 OT on our Reablement and Engagement Team (CORE). Working alongside OT's and Vocational Specialists within aMDT setting. Working at in Central Croydon, easily accessible by public transport.
CORE has 2 intervention Pathways:
- Reablement - working with clients referred by Primary Care staff. Helping develop resilience and reducing dependence on mental health services. Using, individualised goal setting and group work to support Activities of Daily Living (ADL), while also encouraging engagement with social or community options.
- Vocational Pathway - Providing assessments and person centred support for sessions. This Pathway is delivered on a co-working basis with CMHT staff. Vocational Support is for clients who would like to engaged in social or community activities, such as training, education, volunteering and more... as part of their recovery.
You will be part of an established SLaM Croydon OT network and Peer Group. Individual OT supervision will also be provided to identify and support regular training, AHP Preceptorship and CiPD.
Applicants need to show an understanding of the importance of, and commitment to, client centred practice, They must have good communication and organisational skills, and confidence and skills in delivering both individual and group interventions. They must be able to work effectively as part of a team.
Main duties of the job
JOB PURPOSE
To manage a caseload of clients with reablement and vocational needs, using evidence based / client centred principles to assess, plan, implement and evaluate interventions in health, community and home settings.
To routinely provide individual and group interventions appropriate to the clients on the caseload which support their recovery goals, in collaboration with other professionals in the team.
To liaise with Croydon Social services departments and other external agencies including GPs to facilitate clients access, support of needs, physical disabilities and discharge planning.
To provide clinical support to the vocational practitioners in the CORE team.
To participate in the planning, development and evaluation of Community OT / CORE Team services, holding responsibility for defined projects and activities.
To regularly supervise OT students on practice placements and other junior OT staff (as required)
To carry out MDT activities e.g. generic assessments and duty responsibilities.
To work in collaboration with care co-ordinators in other community teams in delivery of Care Plans for individuals under CPA.

The occupational therapy workforce and associated Allied Health Professions workforce consists of approximately 200 staff based in all the Trust Directorates.
The Croydon Community Mental Health teams provide multidisciplinary support and treatment to clients with a range of mental health disorders. Many service users also experience social care and physical health needs. A range of interventions are offered including; Care Co-ordination, emotional regulation, CBT, DBT, OT, Psychology, social work, medical and nursing specific assessment and interventions.
The CORE Team provides both short term Reablement Interventions (goal setting based) to promote resilience, and vocational specific interventions to support social inclusion and occupational and employment opportunities.

1) To undertake occupational therapy assessments for a designated caseload, assessing health and well-being from an occupational perspective. To contribute to assessing and completing risk assessments within the multi-disciplinary team to support a persons recovery. To hold an up to date knowledge of a range of occupational therapy standardised assessments, the models on which they are based and how to carry these out. To identify occupationally focused goals taking into consideration the mental, physical, spiritual, psychological and social needs of the service user and that it is sensitive to age, culture, race, gender, ethnicity, social class, sexuality and disability.
2) In collaboration with the service user, to plan and implement individual or group therapeutic interventions using graded activity to achieve therapeutic goals which support their recovery. To use outcome measures that give clear evidence that occupational therapy has made a difference to peoples occupations and ultimately support social inclusion and promote recovery. To hold an up to date knowledge of safeguarding principles and know how to respond to any safeguarding concern related to the service. To display an understanding of the potential for distress caused by, and social and personal consequences of mental illness for the service user, their families, carers and support networks.
3) To interact with service users, families, carers and support networks in an empathetic fashion and demonstrate a clear understanding of the effects of disability in the areas of productivity, leisure and activities of daily living. To apply a high level of understanding of the effect of disability and provide training and advice on lifestyle changes and adaptations to the service users social and physical environment. To assess and prescribe small aids and equipment and recommend adaptations to maximise independence for service users in home and community environments, where appropriate. To analyse, grade and adapt individual and group activities to meet the occupational performance and participation, environmental and vocational needs of service users.
4) To routinely implement occupationally focused pathways that clearly identify what service users can expect from their occupational therapy interventions whilst working collaboratively with individuals to ensure treatment is targeted to their specific needs. To ensure all occupational therapy equipment and materials are maintained and stored responsibly and advise on resources needed to carry out group and individual treatments including financial.
5) To develop a comprehensive set of skills to effectively communicate with service users and their family using a range of communication and engagement skills. To actively promote relationships with colleagues that support open communication and collaborative working, taking personal responsibility for ones contribution to the process of multi- disciplinary team working. To articulate the meaning and value of occupation in relationship to well-being, recovery and social inclusion to service users, carers and colleagues and the occupational therapy contribution to this. To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards/policies.
6) To use electronic recording systems to document high quality interventions that enhance occupational performance. To participate in local and corporate requirements of data collection. To provide occupational performance data or other reports as required.
7) To participate in service and occupational therapy evaluations, clinical pathways development and implementation, benchmarking and audit activities and to contribute to implementation of service modernisation and improvement projects as required. To actively participate in the delivery of the Trust Occupational Therapy Strategy and via the specific occupational therapy implementation plan in the relevant Directorates. To demonstrate an understanding and application of national guidance and legislation relating to Mental Health service provision in the context of well-being, recovery and social inclusion. To be able to evaluate current occupationally focused research and to base practice on best available evidence. To contribute to the occupational therapy research
agenda by participating in local audit and research projects.
8) To have an understanding of and demonstrate skill in the supervision process through ones own supervision and in the provision of supervision to staff and students. To ensure full engagement in the Preceptorship programme. To represent and act as an ambassador and role model for the Occupational Therapy profession. To participate in the induction, training and education of students and other staff.
9) To develop leadership skills, contributing to decision making about practice using training and development opportunities provided by the Trust. To adhere to the COT Code of Ethics and Professional Conduct and national and local policies.
