About the team:
This is an exciting opportunity to be part of an innovative service within South London and Maudsley. We are recruiting two Community Psychiatric Nurse (CPNs); one for Lewisham Mental Health and Learning Disability (MHLD) team and one for Southwark MHLD. The successful candidates will join exceptional teams that have been rated as 'outstanding' by the CQC. We are looking for two motivated and positive individuals who are passionate about improving peoples' quality of life. MHLD is a clinical team providing a specialist service for adults over the age of 18 with Learning Disabilities and Mental Health needs. The posts involve delivering nursing care to individuals with Learning Disabilities and Mental Health needs in the community. The teams have a range of professionals, comprising of Community Nurses, Psychiatrists, Psychologists, Specialist Positive Behaviour Support Practitioner and administrators who work closely with Guys and St. Thomas' Community Learning Disabilities Team and Learning Disabilities Local Authority Social Services to support people with Learning Disabilities.
Main duties of the job

The post involves delivering nursing assessment and care to individuals with Learning Disabilities and Mental Health needs in the community. To oversee and provide assessment, planning, implementation and evaluation of are to a group of service users within a variety of settings, and ensure records associated with this process are maintained. To work effectively within a multi-disciplinary team and across other agencies, and to provide a high quality of care to individuals with complex mental health and social problems. The post holder will clinically supervise and appraise junior staff and students. There will be an expectation that the post holder will facilitate user involvement in all aspects of service delivery.

- To mentor preceptorship allocated RMN/RNLD/student nurses and joint practitionerstudents. To participate in the induction of new team members.
- To maintain accurate user documentation, establishing priorities, organising time andresources to ensure safe and effective standards to monitor/document care.
- To actively participate in the setting, monitoring and maintenance of evidence-based nursing practice and standards of care, through nursing research and clinical audits.
- Through regular appraisal and supervision, review maintain and promote your ownprofessional development. Undertake regular supervision of own case load with teamleader or other identified professional and ensure this is accurately recorded.
- To act as an accountable professional in line with the NMC code of ProfessionalConduct and with relevant legislation.
