Job summary
We are excited to announce a unique opportunity to apply for a Psychiatry CESR Fellowship at the historic Bethlem Royal Hospital within the Croydon Crisis Home Treatment Team and a world-renowned organisation - South London and Maudsley NHS Foundation Trust.
The CESR fellowship is an opportunity for candidates who do not wish to pursue the usual core and specialty training route to a substantive consultant role (CCT). By the end of the fellowship applicants will satisfy the GMC that their specialist training or specialist qualifications, when considered together, are equivalent to a CCT in psychiatry.
While the CESR training route is not new, our fellowship is. We have created this new post in response to the current experience of CESR trainees across the country who have often felt overworked and underappreciated compared to other trainees. This CESR fellowship has training and development at its heart with the successful post holder having one day of special interest time per week, access to the SpR local teaching and a timetable designed to their specific training needs.
The post holder will be based within the Croydon Home Treatment Team and supervised by the team's Consultant Psychiatrist. The successful candidate will have the opportunity to work in other mental health teams within the Croydon Borough as per their training needs.

Main duties of the job
Role Summary:
- CESR Fellowship for those wishing to apply to the specialty register via the CESR route
- Based at Croydon Home Treatment Team for three days per week
- One day based in a linked team (either inpatient ward, CMHT or Liaison) to achieve all learning objectives in relation to CESR.
- One day special interest day to allow applicants to complete non-clinical learning objectives while pursuing special interests.
- Applicants will be operating at a specialty doctor level and intending to apply for CESR, estimated completion time 2-3 years.

Key clinical responsibilities:
- Management of the day-to-day caseload of the team
- Assessing new patients within the first 48 to 72 hours
- Follow up medical reviews
- On occasion, you will be expected to undertake early discharge assessments on the ward for complex patients with regard to their suitability for Home Treatment input
- Attending daily team handover meetings
- Carrying out risk assessments and formulating appropriate risk management plans
- Formulating care plans and carrying out home treatment interventions as appropriate
- Physical health monitoring of patients including carrying out physical examination and investigations as appropriate
- Liaising with GPs and various other professionals/ teams within and outside SLAM including with Social services, DVLA and the Voluntary sector.
- Attending external patient meetings as appropriate
- Making safeguarding referrals
- Record keeping including preparing reports/letters, discharge notifications/summaries and ensuring that all the relevant information gathered by you following patient contact is recorded clearly in the trusts electronic notes (EPJS) and elsewhere as appropriate.
- Section 12 duties which include carrying out Mental health act and Mental capacity assessments for our patients. Although it is unusual for HTT doctors to attend MH review tribunals, the successful candidate will be expected to attend these if required.
Overall, the successful candidate will be expected to support the team consultant and deputise for the Consultant in their absence.
General/ Managerial and other responsibilities:
- Participation in service evaluation, audit, research and quality improvement
- Participation in service development/clinical governance activities
- Participation in the teams business meeting and any other relevant team/Trust meetings
- Participation in academic activities including team based teaching, case presentation, and teaching medical/other students attached to the service
- Participation in reflective practice sessions
Support, Supervision, CPD and Leave/cover arrangements:
- Regular clinical supervision from the Consultant
- Annual appraisal and job plan meeting
- The post holder will be provided with a mobile phone and expected to be contactable on this during your working hours
- The post holder will also have access to Study leave in keeping with the Specialty doctors guidance and be supported to achieve the CPD requirements within the framework of appraisal and PDP requirements.
- Any annual or study leave will need to be authorised in advance by the service Consultant.
- Specialty doctors are required to register with Royal college CPD arrangements
- The Trust has mandatory training requirements, which the post holder will be expected to comply with in a timely fashion.
