Job summary

The South London Partnership are looking to recruit an 8a Forensic Mental Health Case Manager. The post holder will work across a number of Providers both NHS and independent sector, ensuring forensic patients receive the best quality care as near to home as possible.

The successful candidate will be responsible for the quality assurance of a number of wards in South London. Eight weekly reviews will be conducted and at least one in depth quality review of all units. The case manager works closely with the Provider to ensure high quality care is maintained.The case manager has oversight of any LTS or seclusion and will monitor and review this closely.

The successful candidate will interface with a range of other stakeholders. You will work with other Case Managers in the management of particularly complex cases and delayed discharges (including Transforming Care). You will also work in collaboration with services within the hub to manage relationships with ICS colleagues. This role requires you to work in collaboration with local authorities, third sector and other statutory bodies such as MAPPA, Police, HMPPS, MOJ and CJS in the management of effective usage of the specialised mental health services

Main duties of the job

Eight weekly reviews on all services which include:

  • Listening to and talking with patients, their families and carers via formal MDT discussions and care planning processes
  • Ensuring that the voices of patients and their families are heard and acted upon by the services supporting them
  • Monitoring and reviewing the quality and safety of provision and escalating concerns or issues where necessary
  • Observing providers in practice monitoring and reviewing quality information and local intelligence

Quality Review at least once a year

Recording of all visits on NCMS.

Reporting all reviews to the Programme Partnership Group this includes and concerns and the plan in place to manage any concerns.

Actively manage any units on enhanced monitoring ensuring there is a robust action plan in place.

Engage in the quarterly contracting meetings feeding back any quality improvements or concerns.

Attend all SLP internal quality meetings this would include Quality Matters, Programme Huddles and Quality Governance Group

Improving quality and outcomes

  • The case management of individual patients operates on an originating/placing The case manager of the Host Lead Provider has responsibilities for contributing to the quality monitoring of services within their geographical footprint regardless of whether those patients are part of that case managers caseload. The host case management responsibility is to support the quality oversight of services in a particular geography and to be aware of emerging themes, that may be indicative of a quality or safety concern that requires highlighting.
  • Host case managers must conduct regular provider visits, to ensure quality oversight. These must be at least 6-weekly for CYPMHS and 8-weekly for adults to coincide with patient reviews that involve a physical visit to wards. These visits should include:
  • Listening to and talking with patients, their families and carers via formal MDT discussions and care planning processes
  • Ensuring that the voices of patients and their families are heard and acted upon by the services supporting them
  • Monitoring and reviewing the quality and safety of provision and escalating concerns or issues where necessary
  • Observing providers in practice monitoring and reviewing quality information and local intelligence
  • The case manager will also receive an update on any actions from the annual service review.
  • Annual Quality Review as a minimum SLP are proposing that this is done twice a year. This is a day review conducted by at least two but preferably three case managers and an expert by experience. The case manager will monitor the previous action plan from the review with the Provider. We have also conducted focused reviews in a focused review is helpful when consideration is being given to increasing monitoring or where concerns have been raised.
  • Management of cases that are escalated e.g. escalation calls, liaison with potential providers (involvement will vary across geographies and programmes). Providing oversight of patient pathways, identifying and ensure there are plans in place to address potential/actual gaps in provision and barriers to progress (DToC).
  • Case managers are vital in the delivery of robust, regular oversight of long-term segregation (LTS) for all patients in line with the requirements of the Mental Health Act Code of Practice.
  • Case managers are vital in the delivery of robust, regular oversight of seclusion arrangements for all patients in line with the requirements of the Mental Health Act Code of Practice. The case managers role is important to support national reporting around seclusion and external review processes with the acknowledgement that some seclusion periods could be short term and case manager informed after the event. This is an important function and case managers should be reviewing the overall reducing restrictive practice agenda.
  • Case Managers support a robust and timely response to concerns within specialised services, they work alongside the relevant NHSE/I or LP quality and contracting lead to identify remedial actions where required and ensure that these are completed in a timely and appropriate way. Case managers attend and actively contribute the SLP quarterly contracting meetings.
  • Case managers will receive safeguarding notifications and act accordingly for their caseloads. Where processes fall short of the expected standards case managers will agree remedial action and work with NHSE/I regional and local authority safeguarding staff. These are managed through the SLP internal quality governance structure, weekly quality matters, monthly Quality Governance Group for each Programme which includes reducing restrictive practice.
  • The NHSE SOP also sets out the supervision and support that is required by case managers. In the case of SLP one Band 8b supervisor was transferred in from NHSE to supervise the following:
  • Each case manager will receive at least monthly, one-to-one management supervision with their line manager.
  • Each case manager will participate in clinical supervision with a minimum standard of six sessions per year. Examples of good practice include group supervision and peer review of individual cases.
