Job summary

The Programme Director for Community Transformation will lead a comprehensive and coherent programme of community transformation for South London and the Maudsley NHS Foundation Trust, working closely with local alliance, statutory and non-statutory community partners across health and care. They will be responsible for designing, developing, and implementing service transformation infrastructure and processes to support delivery and evaluation of the Trust wide community transformation programme.

The Programme Director will have full managerial responsibility for the development and delivery of an effective and efficient change programme. This will require the ability to support planning across all areas of the programme, and the ability to influence and deliver successfully in a complex, multi-organisational and multi-professional networked model.

A key focus of the role will include the design and delivery of the modernisation of our Lewisham care offer and supporting estate. This will ensure care is delivered in the least restrictive environment possible linked to the ambitions of our Trust Strategy: Aiming High, Changing Lives.

Main duties of the job

Key Result areas:

  • Lead and deliver large scale and complex programmes of change across the Trust with a focus on delivering a new model of care which delivers on the vision of the NHSE Community Transformation programme and seeks to prevent need for inpatient care, improve community access, improve quality of community care, and reduce inequalities.
  • Lead, develop and maintain robust programme governance arrangements and programme management processes, and the provision of effective, expert advice where needed
  • Direct the delivery of the community transformation programme holding the corporate level responsibility on behalf of the SRO
  • Lead on identifying, developing and maintaining internal and external stakeholder relationships to enable delivery of the community transformation programme and acting on behalf of the Trust
  • Lead the analysis of the strategic and quality case for change, including devising and implementing recommendations to enable significant transformation in the Trust's model of community care and delivery of benefits
  • Lead on the development of the strategic outline case for a new model of care and associated estates requirements in Lewisham as a pilot for the Trust wide new model of community care. This will require collaboration with a number of internal and external stakeholders.
