The Lambeth Living Well Network Alliance (LWNA) commenced on 1st July 2018 and was developed to transform the way that adult mental health services are delivered in Lambeth with the vision that 'every citizen, whatever their abilities or disabilities, can flourish, contribute to society and lead the life they want to lead'.
This is a partnership between Certitude, Lambeth Council, Thames Reach, Lambeth Clinical Commissioning Group and the South London and the Maudsley NHS Trust (SLaM). Together partners support the achievement of the LWNA 'three big outcomes' identified by local people: to support people to recover and stay well, make their own choices and participate on an equal footing in the community.
The LWNA has developed services to meet borough mental health needs including the Single Point of Access (SPA) which opened in Autumn 2019, this is an assessment service which allows people to refer themselves or be introduced for help by GPs or other professionals. People speak to a clinician who refers them directly to the most appropriate support, without having to repeat details. At the same time as the SPA opened we brought community mental health teams, social workers and voluntary sector staff together in two teams. These teams were the Short Term Support Service (who usually support people for up to three months) and Focused Support Service (who support people longer term.)
Main duties of the job
The post holder will deliver, or advise on the delivery of, high quality safe care, ensuring consistently high standards of practice and clinical excellence which comply with their registered professional body and code of professional conduct. As a Mind and Body Community Practitioner you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment. Your professional and personal development will include further postgraduate study and you will be encouraged and supported by your line manager to undertake a Master's programmes. As a senior member of staff working within the LWNA you are required to encourage and foster, collaborative and cross organisational working.

- To hold significant knowledge, awareness and understanding of Mental Health Law, including legislation of particular relevance to the community sector including the Community Care Act (2015) treatment legislation, the Care Programme Approach (2008), the Mental Capacity Act (2005) and Community Treatment Orders (CTO).
- To be personally responsible and professionally accountable for a caseload as part of the community team and lead and manage the work of others as required. Co-ordinating care,communicating with other professionals involved in the care and ensuring regular CPA reviews are held as required. This will include communication with other teams for example in-patient services through attendance at ward meetings and with interface services e.g. Addictions service and CAMHS services where appropriate.
- Independently carrying out assessment, care planning and care coordination with excellent interpersonal skills, ability to listen to others views, respect and value individuals from a diverserange of backgrounds. Have a flexible approach in care provision with a focus on choice and social inclusion opportunities and demonstrate an ability to adopt principles of coproduction when working with service users and carers. To operate from the basis that service users will have an optimumlevel of responsibility for their individual programme of care and with their consent, where appropriate, seeking the cooperation of friends, relatives and carers.
- Recognise and address concerns about the physical health needs of service users with or without long term conditions (e.g. diabetes, asthma etc.); escalating these concerns as necessary to senior members of the multi-disciplinary team.
- Reporting any incident or Serious Untoward Incidents that may take place in a community setting and provide reports and accurate records relating to the incident. Recognise and respond appropriately to self-harm and suicide prevention with particular regard to the risks in community settings, in line with Trust policies and training guidelines. Respond to urgent situations, emergencies or crises using protocols specific to the local community setting.
- Lead and participate in team, locality, LWNA and Trust initiatives as required and promote the strategic aims and objectives of the service and other relevant quality improvement initiatives. Work closely with others in the LWNA to identify the health needs of the practice population and support the development of services in response to those needs.
- Monitor the side effects of medication, utilising evidence-based tools. Nurses administering medication including IM do this in accordance with NMC guidelines and code of conduct and a good knowledge of medication used in a community setting and side effects. Working safely to ensure safe custody of medicines, sharps, and clinical equipment in a community setting; and reporting of all incidents following Trust Policy.
