About the team:

Lewisham CMHT N3 is an integrated multi-disciplinary service, integrated with Lewisham Social Care and providing a broad range of evidence based treatments and interventions for service users with complex mental health needs.

The service recently piloted the new community transformation model in Lewisham, which has now been adopted throughout the borough. The transformation of Lewisham community services, made possible by increased investment in our services, is about enhancing our quality service delivery, improving the experiences of those using our services and enhancing staff morale and in meeting the NHS Long Term Plan.

It is an exciting time to join Lewisham and to get involved in in improved ways of working.

We are currently seeking to fill 3 posts, which are full time 37.5 hours a week.

Main duties of the job

About the role:

We are looking for enthusiastic Community Mental Health Practitioner who will grow and develop within the CMHT N3 at Churchdown. The post holders will carry a caseload of service users in the local community who have complex mental health difficulties, and work with their carers where applicable. The post holders must be able to formulate flexible, robust and holistic care plans with service users and/or carers, to work well under pressure, be experienced in dynamic risk assessment, and to have excellent communication skills.

The post holders will be part of a collaborative, multi-disciplinary team with different professionals sharing diverse knowledge, skills and expertise.

You will use you own knowledge and skills to ensure that service users receive high quality and outstanding care. In addition to bringing your own knowledge and skills, you will ensure that service users access the diverse expertise and resources within the team to ensure that they receive the right care at the right time.

You will empower service users, offering place-based care and supporting them with developing and maintaining community links. Your will work with colleagues within CMHT N3 to facilitate step up and stepdown transitions as appropriate.

About the location:

You will be based at Churchdown Mental Health Centre, 1a Churchdown, Bromley, BR1 5PS.

Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2) and a short ride from the station closest to the CMHT - Bellingham railway station. We also provide services and operate across other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark; and substance misuse services for residents of Bexley, Lambeth, Greenwich and Wandsworth.

  • Mentorship qualification and or Practice Educator / Assessor training)

  • Masters degree
