About the team

Focused Support service offer medium to longer term mental health care to residents in the Borough of Lambeth aged 18 and over regardless of diagnosis using the Care Programme Approach. We are a part of the Lambeth Alliance, and we have a close interface with a variety of SLaM services, other statutory organisations (inc. social care) and voluntary sector.

The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach organisations to deliver an innovative way of providing mental health support for those who need it in the Lambeth Borough; a fully integrated and coordinated mental health system which focuses on people's strengths and supports their whole health and wellbeing.

As part of our journey, to realising a fully integrated system, we have set up 3 Living Well Centres in Lambeth.

Main duties of the job

About the role

As a care co-ordinator within our team there are many opportunities for personal development, and learning opportunities around social care, physical health care, and MDT working, with an opportunity to take part in depot and physical health clinics.

The post holder will deliver high quality safe nursing care, ensuringconsistently high standards of practice and clinical excellence which complywith the Nursing and Midwifery Council (NMC) code of professional conduct.The role includes leading the delivery of comprehensive service userassessment, planning, implementation and evaluation of person-centeredcare in a collaborative way. The post holder will co-ordinate a first classservice that includes assessment and monitoring of physical health,maintaining service users rights, supervising junior staff and students, working collaboratively with the Multi-Disciplinary Team and maintaining a safe and therapeutic working environment and accurate records.

Band 5 nursing roles are only open only to staff with a nursing registration,obtained as a result of completing a qualification recognised by the Nursingand Midwifery Council (NMC).

We are looking for practical and ambitious staff who have a real passion for mental health and a positive "can do" attitude, which will give our service users the best chance of achieving their recovery goals.

  • Evidence of continuing professional development e.g. relevant training (A/I)
  • Experience of working with service users with complex difficulties, including substance misuse and physical health difficulties. (A/I)
  • Experience of working with service users experiencing a crisis related to their mental health. (A/I)
  • Experience of working as part of a multidisciplinary team. (A/I)

Desirable

  • Experience of care coordination / case management. (A/I)
  • Experience of undertaking a service improvement/evaluation project (A/I)
  • Experience of providing guidance to students and/or junior colleagues (A/I)
