About the team:
Lewisham's Community Services are undergoing a radical transformation programme, an exciting opportunity has arisen to take on the role of community practitioner in the Primary Care Mental Health Neighbourhood 4 Team. We have a full times band 6 post.
The transformationwill improve access to our services and will build much closer relationships between secondary and primary care within the local neighbourhoods.
The Mental Health Practitioner will be responsible for carrying a complex caseload formulating treatment, crisis and care plans with the support of the MDT, supporting and engaging in a sensitive manner, communicating with clients and their carers to discuss treatment options and engaging patients to relevant support services, and onward treatment pathways

We are looking for a highly experienced and enthusiastic Mental HealthPractitioner with a hands-on approach who relishes a challenge, has a real passion for mental health and who models the highest standards of care in their practice. We welcome application from BAME candidate.
This is a varied and challenging role and requires an experienced mental health professional (nurse, OT or social worker) with knowledge of working with service users with severe and enduring mental health problems.
You will lead a diverse multidisciplinary mental health team integrating staff from health, social care and thevoluntary sector.The team offers a wide range of focussed interventions to service users with a range of diagnoses, and to their carers.You will provide effective managerial and clinical leadership in the team. You will coordinate and lead on internal and external interfaces and ensure that a continuous flow of service-users is maintained.

The service is base at 1 Southbrook Road, Lewisham, SE12 8LH.
Our Trust headquarters is located at Denmark Hill . We provide services and operate across other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark and substance misuse services for residents of Bexley, Lambeth, Greenwich and Wandsworth.

Key Responsibilities:1. To offer comprehensive initial to service users referred to the service. To manage and ensure the provision of appropriate psychosocial interventions to a caseload of service users who experience transitional mental health problems. To be responsible, under supervision, for the co-ordination and transfer to the relevant Treatment team of a designated caseload of service users with enduring mood, anxiety and personality mental health problems. To ensure that the Trust and Social Services Care Programme Approach and Personalisation policies are followed and standards met.
2. To offer comprehensive on-going assessment and outcome monitoring of all caseload clients. This included consideration and onward referral regarding social care assessments. To ensure that risk is fully assessed and documented.
3. To maintain accurate clinical documentation as stated in the Trust guidelines. To ensure that the care offered is evidence based in line with NICE guidelines. To input data concerning clinical activity as required ensuring that entries are accurate and timely. To ensure that procedures are followed in order for necessary data to be collected e.g. key performance indicators.
4. To ensure that the views of users and carers are sought and incorporated into all care delivered. To ensure that care is sensitive to the needs of minority ethnic service users.
5. To participate in the team assessment and duty rota. To liaise with crisis services and emergency out of hours services. To provide clinical cover for team members in their absence. Ensure effective verbal and written communication with all staff, service users and carers in order to maximise user and carer involvement in the delivery of care and treatment. Liaise as required with other disciplines and organisations to ensure that all issues affecting service user care are managed and communicated to a high standard. To provide written reports as requested.
6. To form effective relationships with inpatient units and SHTT (Home Treatment Team) to facilitate early effective discharge. Maintain and develop primary care liaison with aligned GP Practices as requested in order to assist in maintaining patients within primary care and to develop close working relationships with primary care for service users in the secondary mental health service.
7. To participate in Mental Health Act assessments of service users on your caseload. To be responsible for the administration of medication ensuring that all service users receive adequate information and ensuring that side - effects are effectively monitored. Participate in research / audit / clinical governance as requested.
8. To abide by the NMC/HPC Code of Conduct pertaining to own profession. To abide by the policies and protocols of the South London and Maudsley NHS Foundation Trust and by local Southwark Health & Social Care protocols. To keep abreast of all developments pertaining to the provision of community mental health care. To participate in teaching activities within the team and across community services asrequested.
9. To participate fully in the development of community mental health services as required ensuring that service users are transferred safely and effectively. To undertake other duties as appropriate to grade as requested by the Team Manager or Clinical Service Lead.
