Job summary

The Clinical Team Leader will provide leadership to the Lambeth Crisis Outreach Service (COS) and ensure rapid clinical risk and mental health assessment, formulation, short-term contingency planning and onward referral for people presenting in mental health crisis in the borough of Lambeth.

Working within the community, the Clinical Team Leader will ensure prompt face-to-face assessments and work closely with other mental health services and agencies across the borough of Lambeth, including stakeholders in the Lambeth Alliance. They will act as a manager and a trusted assessor for SLaM and decide on the most appropriate crisis response and treatment pathway for people contacting the Crisis Outreach Service.

Using a range of brief solution-focused interventions, clinicians in COS will empower patients, carers and their families in decision making and divert to a range of health and social care resources for support.

Main duties of the job

The Team Leader will have oversight of the day to day operations of the Crisis Outreach Service and ensure the safe and effective provision of the following

  • Provide rapid assessment and brief evidence based clinical interventions, making autonomous clinical decisions about own professional practice.
  • Responsibility for the development, planning and implementation of brief care and treatment interventions for individual patients within COS.
  • Ensure that individual episodes of care are delivered in a timely, effective and integrated manner.
  • Promote a recovery model that empowers patients, carers and relatives to be at the forefront of decision making and ownership of their packages of care and treatment.
  • Undertake clinical risk assessments, based on latest empirical evidence and compliant with local policies and procedures.
  • Act as the patients and relatives advocate.
  • Enable patients and carers to manage disability, loss and change.
  • Champion patients' rights, including: dignity, equality, diversity, choice and respect
  • Provide timely, accurate and appropriate organisation information to stakeholders and the Clinical Service Lead.
  • Take a lead in the day to day running of the service, in collaboration with the Clinical Service Lead.
  • Participate in regular management and clinical supervision.
  • Monitor practice and development of Band 6 RMNs/ASWs, reporting to Clinical Lead.
  • Conduct appraisal and performance management of clinicians in the service
  • Provide induction of staff.

The Crisis Outreach Service is based at Orchard House, Lambeth Hospital, 108 Landor Road, Stockwell SW9 9NU. Lambeth Hospital is situatedclose to both Clapham North and Stockwell underground stations. It is a 10 minute walk to the busy Clapham High Street which is occupied byshops, restaurants, cafes and supermarkets.Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2). We also provide services and operateacross other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark; and substance misuse services for residentsof Bexley, Lambeth, Greenwich and Wandsworth.
