We are looking for a Home Treatment Team Practitioners to be part of our motivated and experienced teams.
While working with the team you will gain experience in relation to risk assessment, risk management, care planning, engagement and communication with
Clients who are experiencing a mental health crisis and in need of support in their own homes.
You will be expected to provide a high level of care and have excellent communication skills.

The Older Adult Home Treatment teams
The Older Adults Home Treatment Teams (HTT's) provide comprehensive and accessible crisis resolution and home-based care and treatment to people in the acute phase of mental illness in their own homes as an alternative to hospital admission.
The primary function of the teams is to provide intensive treatment in the persons own home or other suitable environment and gatekeep referrals for older adult inpatient beds and facilitate early discharge from hospital.
The HTT's are a multidisciplinary service providing care to residents primarily over the age of 65.
Main duties of the job
The post holder will be a member of the Home Treatment Team. The postholder will also have responsibility of working some night shifts within theIntegrated. Out Of Hours Services which may entail undertakingassessments or offering support across geographical boarders within TheSLP. The main target group are people with severe mental health problemsexperiencing mental health crisis.Home Treatment Team (HTT) is a multidisciplinary, community basedmental health team that aims to provide a safe and effective home basedassessment and treatment service as an alternative to in-patientadmission, and for early discharge from the ward in the least disruptiveand restrictive environment. It works in conjunction with, and is integratedwith, other local service elements. The workload is team-based and teammembers do not carry a specific caseload.
The HTT's are required to gate keep all admissions to hospital.The key features of the service are that it is accessible, mobile, respondsquickly and acts as the gatekeeper to in-patient services and assessmentsfor psychiatric presentations at A&E. It offers care within abio/psycho/social model with emphasis on symptom management, within aRecovery and Wellbeing Model and encouraging the service user (andfamilies) to cope and understand their crisis.
