Mental Health Practitioner

An exciting opportunity has arisen for a full time Band 6 Mental Health Practitioner in the new Low Intensity Team (LIT) in Southwark.

The Low Intensity Team (LIT) will work with service-users in Southwark from a wide range of backgrounds who have stabilised following a significant period of mental illness. The LIT service-users no longer require a high level of support under the Care Programme Approach but are receiving ongoing treatment which can only be provided via secondary care service or are in receipt of a package of mental health care funded by Southwark Council.

The post holder will also work alongside the SUN (Service User Network) which is a service for people whose early life experiences present them with longstanding difficulties regulating their emotions in a way that affects their relationships. These people are often given a personality disorder diagnosis.

The SUN aims to promote a sense of community and assist people in the self-management of their difficulties. In some areas, SUN has been shown to reduce usage of acute and crisis services. Staff will be supported by the wider psychology and psychotherapy staff within the personality disorder pathway who will also provide input as required.

Main duties of the job

The Mental Health practitioner will undertake assessments of need and coordinate the development of sustainable recovery-focused care plans. They will provide practical support and carry out planned interventions to address assessed needs and to overcome barriers to access and social inclusion. Using a strengths-based approach they will support service-users to develop their own solutions, minimising long-term dependence on services through self-directed support and promoting the take-up of personal budgets where appropriate.

As part of the SUN service, the practitioner will co-facilitate a weekly open-access group.
