Community Mental Health Nurse Complex Emotional Needs

Job summary

An exciting opportunity has arisen in the CNWL Community Mental Health HUB for a Band 5-6 Development Nurse within the Complex Emotional Needs Pathway. The service offers help and support for patients requiring secondary Mental Health Services in Milton Keynes.

The post holder will need to have an excellent knowledge of Mental Health and current mental health legislation. Post holders will need to demonstrate the ability to support and apply a holistic approach to assessing and supporting the health and social needs of people experiencing severe and enduring mental illness. Interventions are delivered both as groups and individual 1:1 sessions with a focus on a trauma informed holistic approach to care.

Main duties of the job

The post holder will be responsible for the assessment, development and evaluation of programmes of care using Dialog+ and act as keyworker for a complex case load. The individual will be required to work as part of a multi disciplinary team within a team approach. The individual will be required to undertake specialist assessments as part of professional role.

  • To provide mentorship for students on placement within the team.
  • To manage a caseload of service users with complex needs, using evidenced based / client centred principles to assess, plan, implement and evaluate interventions.
  • To undertake specialist assessment as required.
  • To offer both individual and specialist group treatments and to deliver direct care, according to individual treatment plans and in response to identified need.
  • To maintain up to date written and electronic records of all interventions, to record information and write reports as necessary. To use the integrated mental health services documentation in line with the local policies.
