An exciting opportunity has arisen for a motivated and enthusiastic pharmacist to join the Horton Mental Health Rehabilitation Service at Central & North West London NHS Foundation Trust.
We are looking for a dedicated pharmacist with an interest & passion in Mental Health Rehabilitation services. You will need to demonstrate a person-centred approach and have effective interpersonal and organisational skills.
The post holder will lead on providing a comprehensive, Medicines Optimisation Service, and advocate a patient centred approach to medication discussions.
Supporting people on their journey to recovery, including self-management of medication. As a key member of the MDT, you will attend ward rounds and be an expert source of medicines advice and support, as well as provide medication related training to the MDT when required.
The post holder will also take a lead to drive forward Medicines Optimisation across the wide Mental Health Rehabilitation Service Line.
Main duties of the job
To deliver an advanced comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care at Horton Rehabilitation Service
Promote personalised care and shared decision making with patients, including supporting the move towards self-management of medication.
Be a source of expert pharmaceutical advice to the MDT
Provide input into policy and procedures affecting the use of medicines at both local and trust wide levels.
To liaise with acute Trust pharmacy staff to support an effective dispensing and supply service.
To provide managerial and clinical supervision for pharmacist(s) working within Mental Health Rehabilitation Services

EDUCATION AND QUALIFICATIONS
Essential
- Masters Degree in Pharmacy (or equivalent qualification) + pre-registration training
- Registration with the General Pharmaceutical Council
- Post Graduate Certificate in Psychiatric Therapeutics (or equivalent)
- Post Graduate Diploma in General Clinical Pharmacy
Desirable
- Accredited Membership of RPS Faculty / Credentialed Membership of College of Mental Health Pharmacy
- Non-medical prescriber or working towards
PREVIOUS EXPERIENCE
Essential
- Extensive post registration training relevant to hospital pharmacy including experience providing Clinical Mental Health Pharmacy Services
- Handling complex medicines information queries/undertaking clinical reviews and providing tailored medication advice/ treatment plans for individual patients
- To have undertaken clinical audit and QI projects
Desirable
- Previous experience in providing clinical mental health services.
- Previous experience in general pharmaceutical care and older adults
- Staff management experience
