Looked After Children Nurse

Job summary

The post holder will work in partnership with the dedicated Looked After

Children Health Team and with partner agencies to ensure that children who are Looked After achieve the best possible health outcomes. This includes completing review health assessments, attending multi-disciplinary meetings, delivering training and offering advice and support to multi -disciplinary/multi-agency staff, carers, children and young people.

Main duties of the job

  • To work alongside the Named Nurse, Specialist Nurse and the wider LAC health team, planning and undertaking statutory health assessments and formulating health care plans for LAC and young people aged 0-19 years.
  • To ensure assessments are completed within statutory time-frames and completed reports are produced in a timely manner for quality assurance prior to processing.
  • To contribute to and undertake the training and ongoing development of health professionals, social workers and foster carers.
  • To act as a resource and support, providing expert advice to social workers, foster carers, teachers, residential workers and young people who are looked after. To arrange and attend regular LAC surgery's with Social Care.
  • To arrange for any urgent needs identified to be addressed promptly by referral to appropriate services.
  • To ensure any health issues identified which are a matter of concern are acted upon immediately and that the Social Worker for the case is notified promptly by email and telephone contact.
  • To attend any Looked after Children's reviews if required for complex cases.
  • To contribute to the provision of comprehensive reports in the development of the service.
  • To assist the Named Nurse and Specialist Nurse in maintaining an accurate, up to date database on health assessments, A&E attendances of LAC and a missing persons schedule.

  • Experience of working as a member of a team but also able to work autonomously

  • Management experience or evidence of advanced practice
  • Experience of working directly with looked after children, UASC and care leavers
  • Planning and delivering training (health promotion)

Knowledge

  • Knowledge of childhood vaccination programme
  • Risk management
  • Infection Control legislation and maintenance of standards
  • Policies, guidance and working practices that underpin Integrated working and development of services that aim to improve outcomes for Looked After Children and Care Leavers
